What Causes Fast Heart Beat?

Question: I am a 54 year of female, non smoker, not overweight and never have been. I am 5 ft 6 and weigh 127 lbs. I have a history of SVT, PVC's, PAC's, and MVP and moderate regergetation. I have a yearly echo done on my heart and also have been on Tenormin 50 mg since the age of 20 for the SVT. I have worn many holter monitors, and a few event recorders. The event recorder is how I was diagnosed with SVT. I have been having skipping or double beating, whatever it is, I hate it and it scares me as it is different than the few PVC's and PAC's I have from time to time. I have told my heart dr about them, but he doesn't seem alarmed. I recently wore the holter monitor again, but it didn't seem to do the runs of five or whatever they are called. He thought maybe I was having atrial fib as I mentioned it seemed like it was just quivering and not really beating. I can feel it when it happens in my chest and if I hold my breath it does it for about 15 seconds before it will go back into the right rhythm. It did it again tonight after I went to bed, and I could feel it as I was feeling my pulse in my neck. It was like beat, beat, beat, beat, beat, instead of beat, pause, beat, pause. My heart rate wasn't up, but it was out of rhythm and it scares me because I am afraid it is atrial fib, or atrial flutter, or ventricular fib, or wolf parkinsons something, or whatever else could be dangerous and in runs of threes or fours, or more and the pauses are longer. I am afraid it is getting worse and not sure why this is happening. Do you think I need to worry or need to wear the event recorder since the holter didn't pick it up because it didn't do it while I was wearing it as I figured it wouldn't.

I understand how debilitating it can be to have runs of fast heart beat. However I need to know a few more details

1. What medications are you on?
2. Have you checked your thyroid profile ?
3. Have a noticed any aggravating factor for the fast heart beat ?
4. How long does the bear beat last ?
5. Do you feel like passing motion or urinating once it stops ?
6. Is it associated with chest pain and sweating ?

First, I wanted this question answered by a cardiology specialist which I paid for. I wasn't having a fast heart rate, it was quivering and was like back to back PVC's or like atrial fib. I am on tenormin as I mentioned and is 50 mg and have been on it since age 22. My thyroid has been checked numerous times and is fine.No feeling of urinating. It doesn't last that long, maybe 15 seconds. I am not talking about my SVT, only this skipping or quivering feeling and I can take my pulse and feel it and also feel when it goes back into normal rhythm.

As you can see in my profile, I am a certified Cardiologist. I have seen such issues in many of my patients . Holter might not be of help as most of the times the heart rate will be normal and minute disturbaces will be discarded as normal.

This is how you should go about.

1. Get a thread mill test to see for rythm distubances when your hear rate goes high. It almost always happens where is a structural abdnormality. This is the first step
2. Since you have already undergone many non invasive tests I strongly recommend you to undergo electrophysiological study, it is done by a cardiologist who has been trained in EP study. There might be minute areas of abnormal electrical activity which might require intervention.

I have had the stress test before and was fine. I am afraid of the EP study and how could that help atrial fib or ventricular fib, or atrial flutter or whatever the heck I am having. How would they induce me to have it since SVT doesn't bring it on?What about an event recorder to pick it up?
Do you think this is a serious thing if I choose not to have the ep study done?

EP study will help in indentifying the abberant electrical activity. It is especially dont in cases of atrial fibrillation and atrial fluuter. It is a minimally invasive procedure I think you should go ahead and get it done.

It might be a serious thing if you dont go for it as most of your non invasive tests are normal, but what I can decipher from your history is that you are annoyed with your current condition and you need an answer for whats really wrong with your heart so EP study is the ideal test for you.

Event recorder and Holter monitoring might not pick it up

If I have this EP study done, how would they get my heart to do this skipping thing, or atrial fib, whatever it is? Wouldn't they first have to see it to know what it was before doing the EP study? What do you think is dangerous if I don't have it done? Do you think it is atrial fib, or flutter or just back to back PVC's and PAC's?

Small electrodes will be placed directly onto your heart and electrical activities will be seen on different parts of your heart. Any aberration will be rectified by cautery.

Atrial flutter or altrial fibrillation are fast pulses, since you don't have that I am think you might be having PVC or ectopics

What is aberration and what do you mean it will be rectified by cautery. What is cautery? I also still don't understand how they will get my heart to have these skips being I don't have them everyday. Wouldn't I have to be having them a lot for they to see what it is doing. I know with SVT they can give you medicine to induce ur heart into a fast rate, but not the skipping or fluttering.

1.They will pass a small wire through your blood vessel either through the leg vessel or through the hand, enter into your heart through it, place small electrodes to measure the electrical activity. If they find an are which is misfiring they will ablate it with cautery.
2. Aberrant is deviation from normal.
3. There will look into areas which might be causing this issue in you. Without undergoing an EP study its hard to tell if they will find something wrong or not. If you are having these symptoms then probably they might find that area which is aberrant
