Question: I have had recent blood work several times cause my lymph nodes in neck are swollen. Blood in my urine and now bleeding on & off, extremely tired, high wbc &ANC

Brief Answer: looks like infection Detailed Answer: Hi, Thanks for writing to HCM. High wbc and anc are suggestive of infection. But your symptoms are very varied. Enlarged neck lymph nodes can be due to infection in head and neck region or chest. Blood in urine is usually due to stones. Bleeding on and off (i believe you are talking about menstural bleeding) can be due to uterine pathology. And these all are not related to each other, unless you are having a cancer, which is very rare at your age. So before coming to anything, i would like to know a few more details of your symptoms like 1- are you having any abdominal pain? 2 - any fever? 3- infection/pain in head/oral cavity/neck area 4 - chough or chest pain 5 - duration and progress of your symptoms 6- please upload your blood test results if possible. 1. Yes im having pain in my lower abdomen. It feels like peruod cramps but sometimes goes into my lower back. I have had a fever of 98.9 and 99.9 on and off but nothing higher than that. No infection in neck/ head area. I did have a sinus infection but was on Bactrim for 10 days so even my doctor thought the lymph nodes would fo down after weeks of taking antibotics but nope. I get headaches often, sleep like crazy and feel like im just gonna fall down from weakness and fatigue all most everyday even when i averge 9 hrs of sleep. I have been having this feeling that i have cancer some where but i cant shake that feeling i also sweat when sleeping even though i have a ceiling fan and floor fan on high and house set at 70 degrees and i get itchy legs at night.

Brief Answer: its definitely infection Detailed Answer: Hi, Thanks for the detailed history and the reports. As told earlier, high wbc and anc is definitely due to infection, and your fever adds to it. The question is where is it? The sweating in night is due to fever which you get during the sleep. The headache can be due to sinusitis mostly. And enlarged neck lymph nodes with fever, and recurrent headache point to the possibility that your sinusitis is not cured yet. I usually give my patients with sinusitis, in addition to antibiotics, a decongestant and steam inhalation, to clear the sinuses in an effective way. You need to show to an ENT specialist with an X-ray of your sinuses (ask for an x-ray skull, PNS view). He might change your antibiotics as you have not responded to the previous one. But do take steam inhalations for about 15 mins, at least 4-5 times daily, till it gets cleared. He will also examine your neck nodes, and may ask you to get a needle biopsy of them, if he feels its needed, or may wait to see the response to the newer antibiotic. Also, i will suggest that you get an ultrasound scan of your abdomen, to see for any renal or ureteric stones, and the status of your uterus as on and off bleeding point towards a uterine pathology. Also a 'urine routine' test is needed to quantify the severity of your hematuria and to check of ant signs of urinary tract infection (UTI). As a UTI can also cause fever. The feeling of weakness, fatigue and excessive sleeping are all signs of infection. Though these are also seen in cancer, but as told previously, its very rare at your age. Hope, i have been helpful.
