Question: Last year I was diagnosed with high triglycerides >900. I changed some of my food habits and started doing some exercise. After a while my triglyceride level came to 450 level. Lately, I am having problem sleeping at night, I wake up with a jolt, feel tremors all over the body and most importantly I have started losing weight, I have lost 8 kgs in last 4-5 months without doing anything. My appetite is normal too. I do take alcohol everyday and sometimes heavily. Due to high BP I was prescribed to have a Frunselac by doctor, which I am taking for last one year.

At this time I would certainly evaluate for an entity called metabolic syndrome. This means it is important to evaluate you on a whole to look for diabetes, high waist circumference, high triglycerides, low HDL cholesterol including high blood pressure. Metabolic syndrome can have sporadic symptoms like inability to stay asleep for long hours, and tremors. May be fall in blood sugar levels might wake you in the middle of the night and could contribute to tremors as well. Your triglycerides are still high and warrants thorough evaluation for other reasons for high triglycerides apart from genetics and metabolic syndrome.

I suggest you to see an Endocrinologist in your area to evaluate for fasting blood sugar, thyroid symptoms, kidney profile as well. Meantime continue to take Fruselac which has a weight loss effect including electrolyte imbalance. Weight loss is good but excess weight loss should be evaluated. Similarly alcohol consumption can be deleterious going further and can contribute to high TG. I suggest a rehab program to quit alcohol. I do not see a right medication prescribed for high triglycerides.

All in all poorly addressed medical issues warrants an endocrinologist examination as soon as possible.
