What Causes Elevated SGPT And SGOT Levels Post Child Birth?

Question: Hi. I recently delivered the baby and during pregnancy started having high levels of ALP and moderate sgpt and sgot along with joint pain in knees, fingers, hips. After delivering I waited for two months but none of these improved rather sgpt and sgot increased a lot, 68 and 118. Having vitamin d3 as 27 and no change in joint pains. Also have severe itching. All other blood tests, ultrasound are fine. My ra factor and c protein is 68 and .6 although. What should we do?

I have gone through your history.. Well it looks like pregnancy has flared something underlying which needs to be looked into. I would like to ask you a few more questions before I can guide you further:

1. do you experience any weakness.. like difficulty on getting up from squatting or difficulty on lifting hands

2. is there any swelling in the joints also?

3. any stiffness early mornings?

4. did they check your Esr..cpk and ANA levels?

5. any rashes/oral ulcers/hair fall/ dry eyes/dry mouth

Do let me know these queries. would guide you further from here

1. I don't feel any weakness as such but feel pain while getting up from squatting. No issue in lifting hands. Have pain when I deliberately fold the last phalange of my heart finger , both hands. And pain in left hand thumb where it joins the palm
2. No, nothing visible at all.
3. Yes, stiffness in morning is there. Infact stiffness happens after sitting also. It goes away after 3-4 mins of getting up and about though. My feet, heels, back of heels, everywhere in the feet hurt
4. I have attached a lot of reports which you can have a look, Esr was 68, ANA was negative to be particular
5. Yes , skin rash after i scratch due to severe itching, my gyne said that itching was due to liver for which reports are attached for ur ref , these were supposed to be OK after delivery but haven't since 2 months postpartum

Also, got the ultrasound done for the liver and everything was found ok.

sorry for the late reply. Had a busy OPD

I have gone through your results.

The ESR and Rheumatoid Factor are quite high. Although I could not find the reports of SGOT, SPGT the rest of the liver function seemed normal. CPK levels and the ANA are also within normal limits.

1. The pain in the joints without swelling is common with Vitamin D Defeciency as well or it can be triggered after pregnancy also. Since you dont have early morning stiffness or swellings, it does go against rheumatoid arthritis( even though Rheumatoid Factor is positive). However, one needs to examine your joints to be sure on what we are dealing with.

2. The Raised SGOT/SGPT--. They are not too alarming but if they are on the rise we need to be careful. Fatty liver of pregnancy/Cholestatis is a known complication and can happen after pregnancy. It eventually gets better with time once the horomone levels come back to normal. Incase you are experiencing abdominal pain, decreased apetite we can get the markers for autoimmune hepatitis done. ( Anti LKm Antibody, Anti Smooth Muscle Antibody). But I would suggest you continue with the treatment and wait for a while to see if it recovers. The gold standard test still remains liver biopsy....But since you would be breast feeding for now, we can wait for a bit more as the results are not too alarming yet.

Take Ultracet for your joint pains. Avoid any strong pain killers. If needed take a short acting( Like Brufen) after feeding.

Last piece of info which I forgot to share, I have allergic rhinitis and wondering if Esr is high due to that. Am not breast feeding for some reason as of now.

Can the ra and c protein be high due to rhinitis as well ?

Well, RA can be positive in 10 % of normal population as well. Whether it is significant enough to cause the disease, is something we need to observe and follow up. Just RA factor positive and no joint swelling, we normally dont treat the patient.

High ESR can be due to allergic rhinitis and post pregnancy as well. It takes time to come back to normal self.

I would suggest you wait for some more time and keep monitoring your joint pains and the liver function tests. Incase any swelling arises, do revert back immediately as there is a lot of scope for hitting the disease early and putting it into remission
