Question: I have been having different things going on that I can't explain. They range from dizziness, vertigo, tremors, severe pain in arms and legs. Pain is a burning pain sometimes and stabbing pain other times. I fall a lot my legs give out. I am exhausted some days I can't do anything including gutsy awake. I have numbness in my face. All these symptoms come and go. They will last a week or so then go away except for the leg pain which may be the result of 8 back surgeries.

Brief Answer:
Indoor admission.

Detailed Answer:
Hi,

Thank you for your query.

1. With so many symptoms, and the history of fall, I would admit the patient.

2. Dizziness is an unsteadiness, Vertigo is a spinning sensation. Do you have both? have you had your ears checked?

3. The tremors may be due to weakness and fatigue. The leg pain might be due to multiple back surgeries, or other conditions such as varicose veins, however the arm pain may be due to various reasons such as cervical spine problems, carpal tunnel syndrome, diabetic neuropathy and so on. Have you had any blood tests? have you ruled out anemia? Do you get blackouts when you fall?

4. Face numbness suggests Trigeminal (Cranial Nerve) involvement. Is it on both sides? If it is one sided, it will be better to rule out transient ischemic attacks (TIA). An MRI Brain + Spine and a Neck Color Doppler Study will help. Get a Neurologist's opinion.

5. Are you currently on any medication? Drug induced side effects, deficiencies of vitamins, hormone imbalance including thyroid hormones, peri-menopause changes and increased metabolite levels such as uric acid commonly lead to such symptoms. What are your blood pressure readings?

I hope that I have answered your queries. If you have any further questions, I will be available to answer them.

Regards.

I have had a MRI of brain which showed no mass, few scattered nonspecific white matter signal intensities both cerebral hemispheres that could reflect micro vascular changes or demyelinatinf foci. Also a cervical MRI which showed c6-7 spurring toward left fora imam narrowing left moderate degenerative disk changes. The numbness in face is both sides. Last MRI of lumbar was 2 years ago showed nothing surgical. I don't get blackouts when fall my legs just give out. My ears have been checked are fine.
My medicines are several. I am taking 2 different Meds for diabetics. One for high blood pressure as well as morphine and Percocet and cymbalta for pain.
I am not having any luck getting to see neurologist. It is an 8 month wait

As below:

Detailed Answer:
Hi,

Thank you for writing back.

1. From the above information, you have symptoms on both sides with non-specific MRI and normal ears and no blackouts.

2. Hence you first have to rule out drug-induced symptoms, for example cymbalta and dizziness. It is not possible to suddenly discontinue these medications and hence indoor admission is advised. it will also get you an immediate Neurology reference.

3. The rest of the investigations should be on the lines suggested above.

I hope that I have answered your queries. If you have any further questions, I will be available to answer them.

Regards.
