Brief Answer:
Dehydration and electrolyte imbalance

Detailed Answer:
Hi

Thanks for posting the query; I'll try my best to help you with this problem.

First of all, I would like to reassure you that the dizziness mentioned in the query is not something serious.

Dizziness is commonly noticed during an attack of Upper Respiratory Tract Infection (URTI). The reasons being dehydration and inadequate supply of oxygen to the brain.

Commonly during URTI, the affected individual consumes less water and other fluids, thus causing electrolyte imbalance and dizziness. Secondly, during URTI, there will be nasal block and strenuous breathing. This retains carbon di oxide in the body and reduces the oxygen supply to the brain, inducing dizziness.

In your case, dizziness may not require medicines. You can try the following measures:
1. Drink plenty of warm water, to reduce the congestion in the throat and nose.
2. Do steam inhalation 3-4 times a day for just 2 minutes.
3. Avoid chilled food and beverages, coffee, smoking and fried food.

If the symptoms do not subside within a couple of days, consider changing the antibiotics after consulting your doctor.

Hope I have answered your query; I'll be available for the follow up queries.

Detailed Answer:
Hi

Thanks for writing back.

It's good to know that you are not doing all those mentioned in No. 3 of the previous response. At the same time, I would also suggest you to get your Haemoglobin levels in the body checked up. Reduced haemoglobin in the body can also cause dizziness.

I wish you good health.

Visit ER

Detailed Answer:
Hi

Welcome back

I would recommend you to visit ER, rather than going around to clinics without any relief. I doubt it to be a viral infection. If that was the case, then it should have been subsided within 3-4 days. Secondly, why was the antibiotics started if it were to be only a viral infection.

Hence, I would strongly recommend you to visit ER and get evaluated completely.

Wish you good luck and good health.

Detailed Answer:
Hi

Thanks for writing back

I do understand the situation. Just for your information, the streptococcus is a bacteria and not a virus. If you are still at home, do steam inhalation and drink plenty of warm water, give voice rest and avoid straining of the voice.

Please keep me informed about your visit to the ER.

Request your doctor to change the antibiotics

Detailed Answer:
Hi

Welcome back

I too feel, you require a change in the antibiotics. Request your doctor to consider changing to broad spectrum antibiotics and also, add on a short course of oral steroids to reduce the vocal cord congestion.

If you can do gargle with dilute povidone iodine; it will reduce the bacterial load and thus throat discomfort.

Detailed Answer:
Hi

Thanks for writing back

I do understand your concern and appreciate the positive effort you are making to fight with the present situation. Also, because of your ongoing issues, your anxiety levels will be increased, which will not give you sound sleep.

Secondly, the edema over the vocal cords will also add on to the ongoing stress and make your sleep difficult. Hence, when you visit your doctor, request him to examine your vocal cords.

Brief Answer:
I am glad, I am of some help to you...

Detailed Answer:
Hi

Thanks for writing back and providing all the necessary details. Also, I apologize for keeping you waiting for this long.

1. I really wonder, why, you have been given three medicines to relieve your anxiety or stress (Topamax, Imitrex and Oxycodone). These medicines used for stress, will make you feel weak and leave you dizzy.

2. Secondly, Clindamycin is not the drug of choice for streptococcal infection. Penicillin group of drugs is the first line of treatment and most of them do respond well for this medicine. A combination of Amoxycillin-clavulanic acid has been the first choice worldwide, to treat the Streptococcal infection.

3. In your query, there is no mention of Asthma or Lower Respiratory Tract Infection (evidenced by respiratory distress). So, I doubt whether ventolin would be of any benefit in your case.

4. Omeprazole 40mg to be taken before breakfast, this relieves the Laryngo-pharyngeal reflux and thus edema of the throat and vocal cords.

If I were to treat you, after going through your inputs, I would suggest you the following medications:
1. Diclofenac or Ibuprofen for pain (if there is no evidence of Asthma).
2. Amoxycillin clavulanic acid (an antibiotic, to treat Streptococcal infection)
3. Omeprazole to treat acid reflux
4. Povidone iodine throat gargles or spray (to reduce the bacterial load in the throat)
5. Steam inhalation
6. Voice rest
7. Consuming plenty of warm water

Most of my patients get better with the above set of treatment.

I feel sorry for you and I wish I could have helped you in a better way.

Wish you good health

Reduced hydration is the cause...

Detailed Answer:
Hi

Thanks for writing back

Both of your problems seem to be related to dehydration. Darker urine is seen when there is reduced hydration in the body. At the same time, the stools also get harder and one has to strain really hard to relieve the bulk.

Red color in the urine may be because of the medicines you are taking.

I would suggest you to take plenty of warm water; I can bet this will reduce most of your problems in the body. Also, consume plenty of fresh fruits and vegetables; this will form roughage and helps in easy clearance of the stools. Fruits and vegetables are rich source of vitamins which will boost your immunity.

Hope this answer was beneficial to you.

Hi

Welcome back

The drug is absorbed within one hour and has an elimination half-life of 2 to 5 hours.

You may notice some improvement in your symptoms within the first 2 weeks of your treatment. However, it may take 6–8 weeks to experience the full benefit of oxybutynin.

All your problems seem to be inter-related

Detailed Answer:
Hi

Thanks for writing back

I'm extremely sorry for the delay in answering your queries, as I am on a training program till August 2nd. I'll definitely answer all your queries, but, sometimes it may get delayed, kindly bear with me.

With all the discussions which has happened with you till now, I have realized one thing; all your problems seem to be inter-related.

1. Sinus related problems: Recurrent nasal infections cause blockage of the sinus, thus giving rise to headache.

2. Due to recurrent sinusitis the Eustachian tube gets obstructed, thus fluid in the ears and reduced hearing.

3. Sinusitis can also coexist in people having Migraine. Thus migraine can worsen sinusitis and vice versa.

4. Both Sinusitis and Migraine are stress inducers. Hormonal derangement happens during stress. Hence, the irregularities in the menstrual cycle.

5. During irregular menstrual cycle, abdominal cramps are bound to happen.

6. Also, stress can cause constipation.

Thus, all your symptoms are inter-related. I really doubt whether your neurologist can help you to reduce your abdominal related issues.

Yes, your neurologist can help in de-stressing your nerves.

Please do not panic. The pill would have traumatised the area if crico-pharyngeal sphincter. The area adjoining the food pipe and the wind pipe.

I'm sure, it can't go down into the wind pipe. Have some banana or other soft foods, it will help in reducing the pain by pushing down the pill (if at all, it's stuck in the throat).

Hi

Thanks for writing back

In which part of your body was the yeast infection detected.

Secondly, the pain you were complaining below the left ribs and the back could be due to gastritis. This pain does not get relieved with pain killers.

Instead of Omeprazole you can try Esomeprazole which is more effective than the former.

Prokinetic is required...

Detailed Answer:
Hi

Thanks for writing back

I do feel, we are missing some important clues. You have already been on various medicines but without any relief.

I strongly feel, you should start with Esomeprazole and a prokinetic along with that. You need to take this combination for at least a month. Later on, Esomeprazole alone will suffice. And, the same should be used for at least 3-4 months and gently tapered before stopping the medicine.

After so much of useful discussion with you, I have never come across any symptoms described by you which gives an impression of yeast infection.

A thorough examination by an apt ENT specialist is required.

Get yourself checked before it's too late...

Detailed Answer:
Hi

I'm extremely sorry for the things happening with you. I don't understand why these things happen with you itself. Please get yourself checked before it turns to become worse.

I do wish you recover from all these problems very soon.

If you were feeling better with steroid spray, please restart it again. Most of your symptoms will reduce with it. Also, oral steroids for 3-4 days will hasten the healing process by reducing the edema.

Wish you speedy recovery.

Detailed Answer:
Hi

Thanks for writing back

Normally the discharge from the ear drains into the nose, but for you, there could be post nasal drip which is giving this sensation. If it is discharging from outside the ear, then, its a problem.

With the present medicines hope you will feel better.

Please see the answer below...

Detailed Answer:
Hi

Welcome back

It's good to know that it was just a shoulder sprain. Also, it would be wiser to get the swelling examined in the initial stages.

Secondly, I doubt you are having sugar related issues. You can get the investigations done, and I'm sure, the reports will turn out to be normal.

Don't worry about the backache. That will become normal with good physiotherapy.

Try to de-stress yourself with meditation and relaxation exercises.

I pray god to give you quick relief from all your problems. Good luck.

I do understand that you need to get Fasting Blood Sugar (FBS) and Post Prandial Sugar (PPBS) levels checked. FBS is done in the empty stomach and PPBS one hour after taking the food. This will assess the blood sugars during two instances.

What Causes Dizziness With Strep Throat And Fever?

Brief Answer: Dehydration and electrolyte imbalance Detailed Answer: Hi Thanks for posting the query; I'll try my best to help you with this problem. You can try the following measures: 1. Regards Dr. Naveen Kumar N. ENT and Head & Neck Surgeon
