What Causes Difficulty In Swallowing Along With Dizziness While On Metformin?

Question: For a couple of years, I've trouble with swallowing. It seems worst with cold liquids but even with room temperature water or skim milk at times. Then for the past six months or so I have been having problems with extreme and rapid weakness with my major muscle groups. ( neck muscles to hold up my head, lifting and holding up my arms to shower, brush teeth, standing at sink, and holding up my arms to shave or even lift my arms to apply deodorant. When sitting I need to keep my neck and back supported. This all seemed to start after a hospital stay in April 2017 for excess fluids on my chest causing me to stop breathing as my muscles relaxed while trying to fall asleep. I would arouse gasping for air. After removing about 7 quarts of water I could then fall asleep fine. Added to this weakness seems to be dizziness and light headed. A week ago, I blacked out for a brief period. Then last Friday after a Thursday blood test my VA care nurse called me first thing in the morning and told me to stap using Metformin 2000mg daily immediately. Something showed up (I'm not sure what) that caused alarm about Kidney damage. He told me to keep taking diuretics even though they have not been helping much as I had a couple of episodes falling asleep last night even with my cpap on. I am a Type II diabetic and have Congestive heart failure and coronary artery disease and all the bad stuff that goes along with it.
Can you give me an idea as to what is going on, how metformin fits into the picture, where I might be headed. Thank you very much.
Let me know if you have any questions.

Brief Answer:
Extremely high doses of metformin.

Detailed Answer:
Hello,
Thanks for asking query on HCM.
Have gone theough your detailed narration my dear patient.

Metformin is contraindicated in patients with Hepatic and Renal problems.
Fluid accumulation in body also termed as congestion can be either due to the involvement of heart or kidney or liver.

Firstly Metformin2000 mg per day is overdosing.
Maximum optimal dose of metformin per day should not exceed 1000 mg.
Most likely due to such high doses of metformin your kidney or kiver functions might have detoriated hence your nurse asked you to stop it immediately.
For a patient in acute phase of congestion its better to use insulin initially and once patient is stable we can shift patient on optimal doses of glimeperide or metformin.Much safer options like gliptins are also available now.

I would like to go through the reports of your 2d echo.
What is you ejection fraction?
Is there any RWMA?
Which walls are hypo or akinetic?
Any valvualr leakage?

Waiting.
