What Causes Constant Ringing In Ears Along With Clogged Ears?

Question: My ear has been ringing constantly for over couple of months now.
It did it before, but on and off.
When I'm trying to clean it with a q tip it feels like it has been clogged.
What should I do?

Good afternoon. My name is Dr. Saghafi and I am in the XXXXXXX OH area of the country. My understanding is that you are the good neighbors to the West in one of my FAVORITE cities, Chicago! My wife's fun time city as well...we always try to go at least once every couple of years....DURING THE SUMMER OF COURSE....what an aquarium you've got....lovely....

Though truth be told....I absolutely lapped it up the other night when LeBron put the hurt to your beloved Bulls down at Ohio State....if you watched that game! LOL....

At any rate, your question revolves around a condition we refer to as "tinnitus" which is a ringing or noise in the ear. You say it's been going on for some time now and that it's done this in the past on a more intermittent basis.

So let me ask a few questions for which you can answer me whenever you'd like:

1. When's the last time you had a doctor look into your ears? Not a midlevel practitioner such as an NP, PA, etc. but a doctor of medicine?

2. When's the last time your ears were ENTIRELY without any tinnitus or feelings of blockage.

3. When's the last time they were actually "cleaned" out by a professional and what if anything did they extract (i.e. large impacted waxball, something even worse, or NOTHING AT ALL)?

4. When's the last time you had a hearing test and do you know if your hearing is considered normal in both ears or do you have a baseline problem?

5. Are you on aspirin for any reason? Or do you take aspirin regularly for any reason?

6. Are there any symptoms of dizziness, nausea, vomiting, or vertigo (room spinning or feeling of movement) associated with your current complaints?

The first thing you SHOULD NOT DO anymore is use Q-tips....those things are murder on people's ears and should've been banned along with the booze by Eliot XXXXXXX and the Untouchables back at the time of Prohibition. Unfortunately, some Supreme Court Judge later ruled that it was every American's right under the Constitution to arm themselves as heavily as they wanted with Q-tips not only out their WAZOOS but IN THEIR EARS as well.

I think it was a statement the judge made to correspondent XXXXXXX Rather to the effect of, "I believe that the use of Q-tips is tantamount to our God given rights under the Constitution to Pursue Happiness...each in his own way....and so Q-tips are qualified and protected under the influence of our founding forefathers."

Well, that's too bad because Q-tips more often times than not render it even more difficult for US (doctors) to see and tell what exactly could be going on inside a person's ear when this sort of thing is happening. It could be anything from a plugged ear canal with impacted cerumen to an infection within the middle ear (probably not in your case since you're not complaining of severe stabbing pains...just ringing). There could even be a perforation which could be sealed by clotted blood on one side or the other.

Therefore, that's the first thing. The 2nd thing is you need to get an examination by a qualified individual with the right equipment to not only inside your ear but the tools necessary to get stuff out if it's there.

You can expedite this process by using one of 2 things that I'll tell you about and then, you decide.

One is an ear cleaning kit that they sell at any pharmacy which has instructions in the box as to what to do. Generally several rinses with warm soapy water is all that's needed....you won't necessarily get rid of the ringing but you'll make it much easier for the doctor to see in there. The 2nd thing is that you should instill several drops in the affected of MINERAL OIL once daily. Make sure your ear is up to the ceiling at home....lay on your side and have SOMEONE ELSE place the drops into the canal. Stay put for about 10 minutes.

Again, is this going to cure the tinnitus or anything for that matter? I don't know...maybe, maybe not...but it will make it much easier for the doctor to get in there and disimpact a wax or furball if that's what has to be done.

Ok....I'll ferme la bouche at this point and let you think about things and potentially answer my questions if you'd like....and then, we'll go to the next Round and see what's what......

Thank you so much for this long answer and your time that you have put in to answer my question.

Well, as for your questions, I did have a doctor look into my ear and cleaned it a few years ago. When I say few years ago, that's at least 6-7 years ago. At that time there was a drainage coming out of my ear and each time it comes it would clog my ear again. He professionally cleaned it and told me that my ear walls are having some kind of skin that its normal for elderly...haha...and i was in my early 30's, how is that possible.
I do agree with you on a note for q tips. Unfortunately, even though knowing that its wrong to use it I was still using it.
I dont have any earache, nausea, vertigo, etc., but when showering if water gets in my hearing would be impaired.

Also, since you asked is I have any test results, I do have those, but not pertaining this matter. Few months ago I started feeling different sensation on my left side (tingling, numbness, pins and needles sensations, as well as the pain), together with heart palpitations and chest pains as well.
I went to see both cardiologist and neurologist, and they both run some tests.
Cardiologist did tilt table test, stress with eco, and had me on monitor for 24 hrs. Everything came back fine, except monitor recorded some episodes of my heart rate being higher, 140 and lasted 1.38 min.
As for neurologist he run bunch of blood work, MRI, EEG, and evoc test. Blood work mainly fine except ANA, and diagnostic tests fine except EEG (some spikes on left side, which did not make sense to him), so he ordered MRI of my neck (waiting for results.
I'm attaching some of my blood work for you to see, please let me know what you think....unfortunately there is no option to attach anything here. Let me know if you want me to copy those results for you.

Anyways, thanks for your response back and doesn't sound like an infectious process. It could be that there is a wax plug that simply needs to be removed with professional cleaning.

I would just have someone handcuff you behind your back and then, do positive reinforcement to NOT use Q-tips by putting them in front of you on a table just out of reach of your head or tongue.....that might cure you of the bad habit! You're basically what I like to call a Q-tip Addict. I think there's a society called Q-tip's Anon. HAHA!

So the doctor thought you were old at age 30?? What? Were they 16? LOL....too funny.

I am not finding any attached test results for anything at this time. I agree that your other issues are not likely related to this.

Bottom line is this....I think that you will still need to be seen by a doctor for a potential ear cleaning so I would still do the mineral oil thing. You should probably also get audiometry done if they find your canals to be relatively clean.

I wouldn't worry about spikes on left side...they are probably being over read anyway. Not sure about your elevated heart rate for 1:38. Were you watching the game between the Browns and Steelers?

Hello and thank you for your return reports and the results.

The only thing I saw which was of any interest was that the doctor who ordered the ANA test has the same first name as my younger brother who is also a doctor. My brother, KOUROSH, is an internist and he, I, and our father (who is a general surgeon and still practicing medicine and doing minor surgeries at age 84) are all in the same office! And that's the only thing I saw in the reports.

All tests are normal.

So I return to my question of whether or not you've had your ear looked at to see if the canal is plugged? Have you put the mineral oil in there to see if anything would loosen up so that someone can "powerwash it out" in case there is a wax plug in there? Are you abstaining from using Q-tips as I wanted you to do? And have you had any type of audiometry or hearing testing done in the past couple of years and if so, what does it show?

Are you on any aspirin or any aspirin like substances?

I have never done any hearing test in the past, and yes, I do not use Q-tips anymore.
I'm not on aspirin or anything like that.

Also, I'm putting mineral oil tonight, was busy during a weekend, and will see and ENT this Wednesday.

Nice--- you gave up Q-tips....was it really that hard? LOL....How are your symptoms anyways? Are they improving or changing in some way? Glad that we checked to make sure you weren't on aspirin or any other similar agent that could cause complications with hearing.

Let's see how the mineral oil rinses do. I think if there is a plug it'll make it much easier for the doctor to disimpact it for you...in fact, if you're lucky and there is a plug...it may dislodge on its own. Can't wait for ENT to get in there and get a diagnosis.

I think if they don't find anything then, a hearing test would be indicated which I'm sure they will recommend anyways. Typically, when you go to ENT for any type of a hearing issue they always tend to do a screening audiometry on you anyways and since you don't really have a recent baseline then, this is a good excuse to get one.

Technically they should look in your canals first to make sure you're NOT plugged up before they do the audiometry so we'll see how it goes.

BTW, just so you know....I think the MRI of the brain, the lab tests, etc. were done in the reverse order of how they should've been done...just FYI for the future. In medicine, it's always best to start simple and work up to more complicated levels of testing and diagnostics rather than the other way around, right?

In this case there were literally thousands of dollars of expensive tests which I hope you were not responsible for paying which showed nothing important and which I would've predicted with a fair degree of certainty wouldn't've shown anything based on the rest of your story and how you were presenting this hearing loss problem. So I don't know who went ahead and launched all that testing but it really should've been held off until at least someone took at look in your ear canals.

Having said that I'm glad it all turned out negative and I know you are as well....but just be aware that as the health care system in our country gets tighter and tighter with regard to spending by the insurance companies they are going to start shifting responsibilities to patients so in the future you may very well find tests such as MRI's, CT's of the brain, and high priced lab tests being billed to patients if either the doctors or the patients insist on doing things in a juxtaposed order with respect to what would be considered the logical sequence....first examination by the doctor followed by ordering of big ticket tests...make sense?

Anyways, I wish you the best for Wed.....Ohhh....BTW, very important....and I'm not kidding about this...but if you are married or have a significant other or anything make sure they know that you are mineral oiling whichever ear is going to get it....You can imagine why I might be telling you that but I've found over the years that patients (or their spouses, BF's, etc.) don't appreciate the taste of mineral oil and especially when they're not expecting it....it's slippery, has a very greasy feel to it and could cause some people to literally get grossed out if they think that you've got some kind of secretion exuding from your ear that they just got inside their mouth, on their tongue, or on their lips.....so be warned and warn them! LOL....
