Question: im 35 year old male diagnsed with ankylosing spondylitis by twos eperamatologist, there is a third dr lin chicagoland bu can't see her till december. so i also asked my persomsl dr.of alll my life and got three seperate treatment plans. dr 1=voltaren and 10/325 hydrocodone everyankylosing spondylitisord the voltsren is twice s day 200mg er for life, dr #2 wantd enbrel 1 shot per week for life dr 3 familyu doctor said he thinks 10mg oxycodone one day which sounds easy. since im 35 he doesnt want tylenol in hydrocodone since lab said no inflameaom. he also said it will destroy my liver, and enbrel cancer and diabetes; he says stay on oxycotim for 1 year and do another x ray/mri to see if spreading then no choice but biologicsl agemt like enbrel. i have a 9 month old boy and 2 yea old. i dont want to die in10-20 years and not/die of cancer or new need a liver cause of this and leave mykids edd for kids/wife too guid. what would you do dr 1,2, or 3/ plsn or different plsn cause im scared to make bad decisiom/ or shoud i see pain msnagement doctit?

Well, if you have a confirmed Ankylosing spondylitis you need to start treatment for the same. I will chalk out a plan for you, which is usually the standard protocol we follow..

1. Firstly you need to start with Anti inflammatory( Like Celecoxib 200 mg twice daily) for at least 4 weeks and then start reducing the dose. Oxycodone wont work in your case as it does not reduce inflammation. It will only provide mild relief in the pain but not stop the disease process. SO Plan 3 is out.

2. Once you feel better with pain, start exercising. Swimming is good for you and daily brisk walk is equally helpful. Do some stretching exercises daily

3. If it doesnt get better with this, next line is to add the 2nd line of medications...We use Sulfasalzine here( In US they normally dont prefer) but it does work. IT takes it s slow course of action but helps in the backache and neck pain

4. If all fails, then I would suggest you go for Enbrel/REmicade. It is a very good drug. But it still does not cure the disease, only controls it. So we dont recommend it first line but if all other drugs fail it is helpful then. Dont be scared of it. It does not cause cancer or anything as such. And you dont have to take it too long. We normally give it a course of 6 months and then see how the response is. If your disease gets better with it, we can try reducing it. We have patients here who have been using it for 10 years and they are still ok
