What Causes Cold Sweats And Dizziness While On Amiodarone?

Question: I woke up this morning sweaty and dizzy (room spinning type). I am diabetic and have a heart fibulation history about 10 years ago. I take amiodarone. I have woken up with cold sweats, but this is the first time I have woke up dizzy. XXXXXXX XXXX YYYY@YYYY

I understand your concern and would like to explain that your current complaints (cold sweats and dizziness) may be an expression of a hypoglycemic condition, but may also be caused by a paroxysmal episode of atrial fibrillation, as well as a possible transient hypotension.

- Coming to this point, I recommend monitoring closely your fasting blood glucose and several times during the day for a better understanding of your blood glucose trends, thus concluding on a better review of your anti-diabetic therapy.

- Also, I advise you to closely monitor your blood pressure values (several times daily for a couple of days up to a week) to possibly detect any episodic arterial hypotension.

If such is the case, a reduction of Lisinopril daily dosage should be considered.

- An ambulatory 24 to 48 hours ECG monitoring is advisable to investigate for any possible paroxysmal atrial fibrillation.

As you are 65 years old and also diabetic you have a CHA2DS2-VASc score (the score used to predict the risk for stroke in atrial fibrillation) of 2, imposing you to a moderate- high risk of stroke.

In such case, Aspirin would be insufficient to protect you from this adverse complication (stroke) and oral anti-coagulation is the recommended therapy.

You need to discuss with your attending physician on the above mentioned issues.
