Brief Answer:
Ischemic Heart Disease

Detailed Answer:
Hello

Thanks for the query

Shortness of breath on exertion and chest pain is usually seen in patients with a past history of a heart attack. It is because some of the muscle fibres of the hear would have gotten damaged. In these patients the exercise tolerance is a little lesser than normal individuals. For example they become breathless after climbing one flight of stairs. Here is what you should do

1. Approach a cardiologist and get a threat mill test done
2. Get a Pro BNP test done to rule out heart failure, if it comes back as positive then putting you on diuretics will help
3. Adequate control of blood pressure and salt restricted diet is of most importance

I hope I was of help, if you have any further queries please get back to me. My next reply to you will be around 6:30 pm GMT as I am going to the hospital now

Regards
