Question: Hi My son has enlarged adenoid. Always mouth breathinh, itchy ear,itchy eyes. Chest congesion, wheezing. He takes inhaler seroflow 50 mg for a year and some time take levoline incase of cough. He is 5 years old now.How to get rid of adenoids

Better to wait for normal regression of adenoids.

Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Adenoids are lymphoid tissue and are at their peak size at the age of 5 years.
So your son is having adenoid hypertrophy.
Normal adenoid regression occurs till 7-8 years of age.
So after 5 years of age, adenoids fade till 7-8 years and then disappear.
So if adenoids are still present after the age of 8 years then surgical removal (adenoidectomy) should be done.
So at present, in your son's case , we have to wait till the age of 7 years.
Common causes for adenoid hypertrophy are
1. Viral Infection
2. Allergy.
So better to prevent recurrent viral infection by annual influenza vaccination.
And for allergy, along with seroflo, he should start antihistamine and anti allergic drug.
So wait for spontaneous regression of adenoid.

Hi,

Thanks for your reply. For how long he needs to take antihistamine and anti allergic drug? What are the names of the medicines you suggest?

From the age of 2 years he was investigated with enlarged adenoid. Initially he used to have severe dry coughing ,wheezing,tremendous itchy ear. Then he used to take nubulizer several times as per doctor's advice.Then last yesr XXXXXXX 2014 he was advised to take inhaler for three months. But it looks like if I stop he gets cough back.

At night he has mouth breathing and he has a very disturbed sleep and even no one can sleep with him as he moves from one side to other side of bed continuously. His nose is always blocked. Since 3/4 years we are facing the problem. We really want a solution now.

a) Initially I gave him Meta spray for few months. What do you suggest to start that again?

b) Would there be any possibility of getting affected his hearing capacity?

c) Would there be any homeopathy treatment /Home remedy/Ayurvedic treatment?

d) As you know Swiming is a good exercise for kids who suffer from cold/cough. But I am not sure how good it will be for him. Please suggest if swimming can help him for this.

Hello dear, thanks for your follow up question on HCM.
I can understand your situation and problem.
Antihistamine name is levocetrizine.
Anti allergic name is montelukast.
Both are available in combination and specifically in pediatric dose. Both are safe for long term use in children. So better to start these drugs. Usually these drugs should be given for 1-2 years along with inhalers. And reaponse is seen after 1 year. If patient is improving then we can gradually taper down.
By your history and description, in my opinion your child is having allergic rhinitis as well as allergic asthma.
And the best treatment of allergy is identification of allergen and its complete avoidance. Allergen can be anything like dust, ,pollens, smoke, fur etc. So better to avoid such things.
Answer to your 1st question.
Better to use Fluticasone and azalastin containing nasal spray. It will give excellent results in blocked nose.
Answer to your 2nd question.
Very less chances of hearing problems. But position of adenoids is near the eustachian tube. So enlarged adenoids can cause hearing defect. But the incidence is very very low.
Answer to your 3rd question.
Honestly speaking, I don't believe in alternative medicine. So better not to start them. Yes, you can definitely give him steam inhalation 5 - 6 times a day.
Answer to your 4rth question.
Better to avoid swimming because swimming can cause upper respiratory tract infection ( URTI ) easily. And this can actually aggravate the condition. So better to avoid swimming.
Hase he ever tested for
1. Total IgE level
2. Absolute eosinophil count?

Normally I used Otrivin,Rhinitis nasal spray for my 5 years old son.

As you have suggested to take nasal spray which contain Fluticasone and azalastin spray- Is it a steroid containing spray?

1) What is the name of the spray which will contain both (Fluticasone and azalastin)? is it safe for 5 yeras old kid?
2) For how long he needs to take this spray.

3) Could you suggest any ear drop to reduce ear itching?

4) Does steam inhalation increase wheezing?

He didn't do any test(Total IGE level, Absolute eosinophil) which you have mentioned in your pewvious email.

5) I am based in XXXXXXX Do you know any doctor in XXXXXXX who treats adenoids very well. If you know any one please let me know.

Avoid otrivin.

Hello dear, thanks for your follow up question on HCM.
Otrivin is having xylometazoline. It is habit forming nasal spray. Better not to use it. It is not good for pediatric patients.

Answer to your 1st question.

1. Name is Duonase or Combinase, whichever is available. Both are having Fluticasone and azalastin. It is safe for children and first line treatment for allergic rhinitis. Fluticasone is steroid.

Answer to your 2nd question.

He needs to take this spray for atleast 6-8 months and then we can taper down its use after assessing improvement.

Answer to your 3rd question.

Is he having redness or infection in the ear? If yes then better to start candid ear drops. And if only irritation then staet soliwax ear drops.

Answer to your 4rth question.

No, steam inhalation will not cause wheezing.
And total IgE and absolute eosinophil count are the markers for allergic disease. So get it done.

Answer to your 5th question.
