What Causes Burning Sensation In Anal Canal And Pain During Defecation?

Question: Dear doctor,
This is XXXXXXX XXXXXXX from XXXXXXX I was completly fine 3 months back.
Around 3 months back i started working out in a gym. Since am a complete vegetarian, I started having sprouts and fruits for my breakfast. I also used to have a glass of milk along with protein powder (Modicare) twice daily. In addition i used to have raw vegetables like cucumber carrot regularly. Around a week later, I started having severe flatulence (non foul smelling) which was very frequent and was embarrassing, irritative especially during my working hours. Later i started having burning sensation in my anal canal which increased and started paining. Pain was very severe during defecation. I stopped all my protein supplements. I started on Bakson Homeo #B 9 but with very little relief. Later when it was excruciatingly painful i also started on Lignocaine local gel and PILEX oint.. which gave me little relief. But my routine has become very painful. I have observed recently that, even if i take very little protein which is used in normal preparation of food am having increase in pain. It decreases on eating only curd rice. now a days am having pain even while sitting and walking. My anus has become very sensitive and painful.
Other than that i dont have any problem. My stools are not very hard not very loose(might be due to Bakson homeo B9 ). I pass stools atleast 3 times per day. I dont have fever.
Am going into depression due to pain and burning. Kindly help me out.

Thanks for your query.
I have read all the information given by and and understood the problem you are facing. I can really understand the pains and depression and irritation you must be going through and also a thought why me / why should have all this started when you started the workout and proteins in all veg forms.

I would suggest the following treatment in such a case:
-To stop anything you started new in the last 3 months.
-To stop milk and all the milk products including curds.
-To start on the normal / regular diet exactly the way you were taking 3 months back.
-Sitz bath > to sit in a large tub with warm water. Do not add anything to the water, to be done at least thrice a day.
-Irrespective of the cause, to take a course of an antibiotic and metronidazole for 5 days. This is necessary as there will be either a primary intestinal infection and / or secondary infection in the anal fissures.
-Take a painkiller, pain has to be under control, you may take the tablets which suited you in the past.
-Also add Pre and probiotic, activated charcoal tablets and multivitamins. Additional medicines as per the advice of your Surgeon / Gastroenterologist.

I hope you have been actually examined (per-rectal examination) by a General Surgeon / Gastroenterologist. Once the things are under control a colonoscopy should be done.

See the results after a week, get a review and continue the further treatment
if needed.

The idea is to get a relief first, then the other things. In the meantime get all your blood, urine and stool tests and an ultrasonography of the abdomen if not yet done. Please remember a dictum- the more stress / anxiety / depression you have, the less will be results. So it is imperative to have these things well under control, request your Doctor to add on the medicines for this to if you need to.
