What Causes Burning Sensation During Urination After Unprotected Sex?

Question: About 4 weeks ago I had a brief moment of unprotected vaginal sex during a one night stand. By brief I mean penetration was less than 20-30 seconds.

On Wednesday of this week I started to feel a burning sensation when I started to urinate and shortly after and stinging inside the shaft of my penis that comes and goes throughout the day. I've also noticed that the shaft of my penis feels hard or swollen from time to time even though it is not erect. I have also noticed some minor swelling in my groin area which I suppose are lymph nodes.

I have not seen any visible signs of an STD in terms of sores or anything. I went to urgent care and they prescribed me 7 days of doxy and did a urinalysis which was clean and a swab of the tip of my penis as well. I'm waiting to hear back results from the swab and urine test.

Does this sound like an STD like Herpes? Or could this be from something else?

I have gone through your query and I have understood your concern.

Burning sensation during and immediately after passing urine and off and on stinging sensation is most likely due to urethritis.

Recent unprotected sex could be a cause for these symptoms.

Urethritis can follow unprotected sex and the likely causative organisms are chlamydia, trichmoniasis, mycoplasma and ureaplasma.
Gonorrhea is unlikely because it is usually associated with purulent urethral discharge.
Though herpes usually presents with visible sores, burning sensation along urethra may also be part of a prodrome of genital herpes.

These all are possibilities.

You are already taking treatment with doxycycline which would cover for chlamydia, mycoplasma and ureaplasma. I suggest you to complete the doses i.e twice daily for 7 days.
If I was the treating doctor I would have also added metronidazole 400mg thrice daily for 5 days to cover for trichomoniasis.

The swab test that they took was most likely done to test for gonorrhea and chlamydia.

Besides I would also like you to get tested for genital herpes.

Thank you for the reply. I will contact my doctor tomorrow when the office opens to see if they tested for herpes. I had a follow up question:

Over the weekend my symptoms have worsened a little bit. The stinging in my penis has gotten more painful and the tenderness swelling in my shaft is more consistent. Also when I touch/examine myself I sometimes feel the stinging inside the shaft of the penis. I have also started to see a small amount of discharge from the tip of the penis. It is clear liquid, odorless. The doxy does not seem to be having any impact yet and I have 6 doses left I still do not see any visible sores or lesions. Does any of this information give you a better idea of what this could be?

Could it possibly be irritation from a masturbation cream that got into my urethra?

I suggest Rx with a single dose of Azithromycin and Metronidazole

Detailed Answer:
Hi.

Urethral burning and clear discharge are signs of urethritis.
Chemical urethritis due to soaps Or creams would usually resolve within 2-3 days without any treatment instead of worsening.

Persistent urethritis after doxycycline treatment might be caused by doxycycline resistant Ureaplasma or Mycoplasma Or it could be due to Trichomoniasis vaginalis. Chlamydia is very responsive to doxycycline as well as Azithromycin.

Therefore in case of persistent urethritis I would suggest treatment with a single 1 gm dose of Azithromycin in addition to a single 2 gm dose of metronidazole

Test results would add a lot to the understanding in your case. Let me know the report as it follows.
