default
Posted on Thu, 20 Aug 2015
Question: When I sit either with feet hanging or up in recliner, I have a burning pain behind my knees. They are so sensitive that if there is a fold or bunch in my pantleg that touches the back of the knee or if there is a bunch in my leather couch. I have the same pain in my heels. I relieve the pain by using a frozen water bottle. Help! What is this?

Brief Answer:
Neuropathy

Detailed Answer:
Good evening. I'm an adult neurologist and what you're describing best fits the mold of a NEUROPATHY of a sensory type meaning that the burning sensations are either the only or predominant symptoms which reflective MALFUNCTION of your nerves in your legs...strength is likely preserved from what you're saying.

Now, to the cause of the problem....well, there are NUMEROUS THINGS that could cause this. Diabetes, Vit. B12, folate, Vit. D, and thyroid deficiencies, and other chemical disturbances (amyloid deposition) in the bloodstream are all potential reasons for your symptoms.

Medications themselves could cause these symptoms as can electrolyte disturbances and of course, liver and kidney failures are in the mix. Alcohol consumption either by history or currently is also a known contributor to these types of burning sensations.

Also, narrowing of the spinal canal in the back or bony spurs that are irritating exiting nerve roots can also cause these symptoms and in those instances, orthopedics or neurosurgery would be the best services to see---BUT if I were you I'd get to a neurologist first following perhaps by a vascular person who can tell you if you might have a reduced blood flow going to the nerves in your back that could also potentially cause this picture though it is much less likely than anything else I've mentioned so far.

As you can see there are a number of possibilities that could lead to this picture and you really could use someone who is organized, knowledgeable of this type of thing (such as a neurologist) so that the right tests can be ordered expeditiously, and diligent so that they will put the whole thing together from isolated or fragmented data to make the call for you. Be prepared though....often times symptoms such as what you describe can occur in the absence of any hard core testing or evidence and the pains and so forth end up occurring without obvious cause or diagnosis. It doesn't mean the search for a cause has to stop if nothing's found....it's just not in the more common categories we typically start looking into when confronted with this type of case.

I hope this answer satisfactorily addresses your question. If so, may I ask your favor of a HIGH STAR RATING with some written feedback?

Also, if there are no other questions or comments, may I ask you CLOSE THE QUERY on your end so this question can be transacted and archived for further reference by colleagues as necessary?
