Brief Answer:
may be prostatitis

Detailed Answer:
HI, thanks for using healthcare magic

It is possible that you may have prostatitis which is inflammation/infection of the prostate gland.
It can be acute or chronic but if your symptoms have recently started then it is likely acute prostatitis.

The prostate is involved in the production of semen. It produces a fluid that makes up 30 to 40 % of seminal fluid.

This can be determined by physical examination and also urine analysis . Imaging of the prostate and abdomen by ultrasound may also be suggested.

Treatment involves the use of antibiotics.

Painful ejaculation may also be related to other structures involved in either the production or passage of semen eg the testes, epididymis or urethra.

To determine the exact cause you would need to see your doctor who would be able to examine you and perform the necessary investigations.

I hope this helps, feel free to ask any other questions

This has been going on for about 5 yrs. After ejaculating the head of the pens getS red splotches and around the head skin. In about 2 days it's back to normal. Also sometimes my left testicle hurts a dull pain like it's bruised.

Brief Answer:
chronic prostatitis or epididymitis

Detailed Answer:
HI

Since the symptoms have been on going for 5 years then you may have chronic prostatitis as mentioned previously.

Some of the other infections mentioned can also be chronic eg chronic epididymitis- persisting infection of the epididymis (also involved in semen production)

You need to consider visiting a urologist who would be able to assess your urogenital tract.
Abdominal examination and examination of the genitalia would be needed.

Imaging would definitely be needed
