default
Posted on Mon, 21 Jul 2014
Question: I just discovered a few days ago a running bruise like rash or fungus on my left buttocks. Looks like a bruise. Starts at top inner buttocks and runs down side of buttocks. Some areas are dry. Looks like a rug burn but I have not fallen. I just returned from Punta Cana but really did not notice this.

Brief Answer:
? Contact dermatitis/ ? Fungal infection

Detailed Answer:
Hello,
Welcome to healthcare magic.

I understand from your query that you have recently noticed a rash on your left buttock.

From your history and description, I would consider the possibilities of a fungal infection and contact dermatitis. ( irritation/ allergy due to some topical applications)

Fungal infection needs to be treated by applying 1% Clotrimozole cream twice a day for 2 weeks.

If it is a dermatitis due to some topical application ( to which you probably were allergic to), you could apply 1 % hydrocortisone cream twice a day for 15 days.

Since the treatment to both of these conditions are different, it would be better if you could upload a clinical photograph so that I can assess better.

Brief Answer:
Eczema

Detailed Answer:
Hello,

Thank you for uploading the clinical pictures.

I have gone through them.

From its appearance, I would consider the possibility of a dry eczema. This could occur as an age related process when the skin lipid ( fat) content goes down and it becomes dry. The dryness can be compounded by application of topical irritants/ scrubbing, etc. I see from the picture that the skin surrounding the rash is also very dry and xerotic.

You could follow the following suggestions-

1. I would advise you to apply 1 % Hydrocortisone cream 3 times a day on the rash for 10-14 days.

2. Apply a good moisturizer, preferably containing colloidal oatmeal ( e.g - Aveeno) 3-4 times a day all over your legs. ( You could apply the moisturizer first followed by the hydrocortisone cream).

3. Shower using only lukewarm water. Hot water could strip the skin off its lipids and further dry the skin.

4. Use a mild cleanser like Cetaphil cleanser to bathe.

5. Apply the moisturizer within 3 minutes of bath so as to trap the moisture.

This should help you to clear the rash.

Hope this helps you.

Take care.

Regards,
Dr. Johny Chacko

Brief Answer:
Could be related to the scrub

Detailed Answer:
Hello,

It could be related to scrubbing. Your skin seems to be very dry. Scrubbing would have irritated the skin.
Hope the suggestions help you.

Take care.
