This may be called as hematospermia and to proved that you need the following:
Send the next ejaculate for tests.
Tests of blood: CBC, PSA is prostate specific antigen, sugar ...
Tests of urine.
Per-rectal digital examination for palpation of prostate and per-rectal and abdominal ultrasound, particularly of genitourinary area.
Ultrasound and color doppler of the scrotum as well as clinical examination by a General Surgeon or an Urologist.
All these measures are done to rule out any sinister diagnosis.
The causes can be:
Idiopathic, that is where the cause can not be found by any means.
Infection in the genital parts.
Malignancy has to be ruled out.
