What Causes Bowel Incontinence And Loss Of Appetite After An Enema Process?

Question: My grandfather is 80 years old. His He is suffering with BPH and using Upper catheter (direct in bladder) due to unable to urinate since last 4 months.
Since last 2 months he is facing problems of constipation, loss of appetite, indigestion, gas.
Doctors did Enema to clear his stomach in December 2015 but even after problem continued.
Again doctor tried to make the colonical test but his stomach was not empty, they did the Enema process on 15th January 2016. After 2 days of Enema he became too weak to doctor deny for the test. His weight is just 39Kg.
The major problem he is facing - frequency of bowel movement is very high (6-8 times daily) and he never feel about it. He lost his control on bowel movement. Everytime with passing of gas, small amount of bowel also pass.
He is using below medicines currently:
1. Eslo AT (for BP. 1 Tab daily)
2. Geriflo D (for BPH - at bed time only)
3. Rifagut 400 (2 tablets per day. For 10 days only).
4. Zinetac 150
5. Gnorm (2 tablets per day for 10 days. using since last 5 days)

Please make clear the pattern of bowel movement as initially you have mentioned that he is having constipation. But in the later part you have mentioned that his stool frequency is very high. Please clarify.

Transition from Constipation to uncontrolled loose motion is a big story which is as below:
Since October 2015, he started facing constipation. Stomach was becoming empty. In the interval of 2-3 days, he used to take medicine to make the stomach empty. On the following day, he used to have loose motion.
During the constipation, he wanted to go to bathroom for bowl movement but he succeeded hardly.
Due to this, he lost his appetite and became weaker day by day.
Gradually what happened is - he started going to bathroom many times - around 15 times but amount of bowl was very less.
We visited to hospital and doctor advised for Enemia therapy (in December first week) as per them, stomach was full of bowl. They did this and even after 2-3 days, loose motion was continued. So, we gave Oxfloxacin Trindazole Tablet to control loose motion.
This medicine didn't work fully and again he had the tendency of bowl movement while amount of bowl was either very less or no.
We contacted with our physician and he again gave the medicine to make the stomach empty.
After this, bowl movement became too frequent and sometimes in his cloth. He started loosing the control in bowl movement.
No one medicines worked except VSL#3 which didn't suit him. So, we stopped that medicine.
We consulted to gastroenterologist on 15th January and he advised for colonoscopy for intestine examine and for this diagnosis, stomach must be empty. Again Enema therapy advised for 3 days but doctor stopped it after 2 days as my grandfather was too weak to survive for next day Enema and doctor denied for this examination.
Doctor gave one medicine (lactohep syrup) to take bedtime everyday to make the stomach empty. It was working. He used to go to 2-3 times if stomach became empty otherwise he has to go to bathroom multiple times for bowl movement.
Appetite also increased and used to take Rice and Khicri with few boiled vegetables.
One day, he took small amount of milk and Tea after one interval of one month.
Problem again started. Frequency of bowl movement increased and it was loose motion. We stopped the lactohep syrup. Gradually he was loosing the control on bowl movement and doesn't know about this untill he clothes become wet.

We contacted with that doctor and he changed the medicine as below:
Rifagut 400, Zinetac 150 and Gnorm
( I have uploaded the documents and reports)
Appetite has decreased little bit and loose motion has been controled little bit too.
Still bowl movement frequency is 6-8 times and he has no control. If goes to toilet, there is no bowl movement but in it happened in clothes.
Since he is using upper catheter and too old, he is unable to go anywhere else and we are contacting to you. We don't know how to handle this situation?

Give him tablet loperamide

Thanks for the details. I have gone through all the reports you have uploaded, all of which are near normal except the ultrasound abdomen

Presently his problem is increased stool frequency and has no control over them.

I will advise you to give him tablet of antisecretory agent like loperamide at present. It comes as 2mg tablet. Start with once per day and then according to the response titrate the dose. It can be increased up to 3 to 6 tablets/ day.

In case he become constipated again then stop it and give syp Lactihep.

So you have to observe bowel pattern and accordingly you have to give tablet loperamide or lactihep Syrup.

It is available OTC

For how many days should he take this medicine? Gnorm will end in next 4 days. Is there any medicine to reduce the weakness?
Any suggestion about food and drink?
Since his First Enema therapy, he is complaining about pain in his left feet which has been increased since last weak and due to this he is not able to sleep quitely in night. Any suggestion about this?

He should be given this medicine till he requires that as per pattern of bowel movement

You should give him high protein, fresh fruits and vegetables for weakness. May add some protein supplements

Avoid milk

There is no relation with enema and pain in left foot. For relief in pain can give some analgesic and at least for once discuss this problem with your doctor who will also examine him physically
