What Causes Bouts Of Diarrhea And Vomiting?

Question: 3 bouts of the following 6 weeks apart:
Sudden onset vomiting and diarrhea with hot sweats and lethargy
Very sudden/violent
Have called ambulance and been given ondansetron. Fine after that until the next time
Have done physical work just before and in some cases have had my head down

Allergy/Celiac has been pretty well ruled out
Blood tests on all organs has been fine
No evidence of bacteria in the gut
I have been given tests for Carcinoid syndrome but the symptoms don't match.

I was interested in what you thought about possible middle ear issue maybe being the cause. As I have been doing lots of physical house or garden work often before these incidents often with my head down. Other things that seem to fit a bit better are gastroparesis or something called cyclic vomiting syndrome. What are your thoughts?

Such diagnosis to consider after ruling out organic GI problems

Detailed Answer:
Hi back,

1. Menier's syndrome (or middle ear problems) are often giving spells of dizziness and vomiting. Diarrhea is not typical for these problems. However, if I was your caring doctor would rule it out after ruling out the other organic issues as mentioned earlier.

2. With regards to cyclic vomiting syndrome, it is also associated with vomiting without any known cause. Furthermore, there are some factors that could trigger it including: chocolate, physical exhaustion, etc.

Once again, my opinion is to run upper endoscopy and/or barium examination and MRI of abdomen to study the motility and damages of gastrointestinal tract.

3. Other possible causes include:
- cervical spondyloarthrosis,
- thyroid problems
- neurological problems

to be rule out through neck X-ray, thyroid function test and neurological examination respectively.
