Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Female/45 - symptoms with bowel movements - itchy anus - sometimes blood streaks on stool or tissue - slight stinging pain - wants to know what this can be...
As per the history you have provided, this looks to be a case of fissure-in-ano.

Please give additional history:
Since when have you got these problems?
Any hard stools or have to strain on passing bowel movements?
Is this a recurrent problems, meaning healing and coming back again and again or for the first time?

Please give additional information for me to assist you better in diagnosis and management.

Brief Answer:
Consult a General Surgeon for examination, confirmation and Rx

Detailed Answer:
Thanks for your feedback.
This occurs classically in fissure-in-ano cases.
I would advise you the following:
High fiber diet.
Avoid all the foods and beverages and stress-like conditions which gives constipation.
Laxative for softening the stool- never allow the dose too much to cause liquid stools.
Sitz bath- very important- just before passing a stool and after passing a stool.
Clean the area very clear of the fecal soiling that remains after cleaning; use cotton soaked in the same water to clean.
Add probiotics and multivitamins.
Avoid stress and mental tensions at any cost; this is a very important factor.
5 day course of an antibiotic on prescription or only if felt necessary by the General Surgeon who should examine you to give you the confirmed diagnosis.

Continue this for 3 weeks and most probably you may not need surgery.
If the results are positive you have to continue doing all this and take proper care. If the medical line fails to relieve the problem you may need a very small procedure as per the choice of the General Surgeon or Proctologist if available in your area.

I hope this answer helps you fully.
