Question: I have been on buprenorphine for pain for 7-8 weeks ago.switched from morphine. I also take klonopin for a year and lunesta for sleep. I started feeling not well about 4 weeks ago with bladder spasms (ic) and over all not well. was given anticholinergic medications and pyridium. and my alt, ast and estrogen came back elevated last week. I am itchy, have red palms. Light green stools, and this morning dark yellow urine. My dose of buprenorphine was .36 daily. I stopped anticholinergics, pyridium, all tylenol and we reduced the buprenorphine in half on saturday. But I was just told it is the toughest opiate to detox from. And I am REALLY feeling the withdrawals. My prescribing doctors are pain management and psychiatrist. Neither haver ever used buprenorphine before it was like a last hope.

I have general anxiety disorder, chronic 24/7 nerve and pain from anklyosing spondylitis. My liver does not metabolize properly through the p450 and get terrible side effects from all medications. I dont and mever have abused drugs. Don't metabolize antidepressants or gabapentin etc.

I understand how distressing it is for you to bear all the pain.

Unfortunately, almost all the pain medications are metabolised by liver.
Since your liver functions are already affected, any pain medication may have undesirable side effects.

You have been using buprenorphine for pain relief. Yes, it is very tough to withdraw from this drug.

In my opinion, hydromorphone or fentanyl are relatively better in cases with liver dysfunction.

Also, tramadol can also have analgesic and anticholinergic effects.

It would be better to go for frequent liver function tests.

Its strange and unfortunate that you could not tolerate gabapentin because it is renally excreted and not metabolised by liver.
You can try pregabalin instead, if you did not tolerate gabapentin.

Itching, green stools, dark yellow urine are all due to impaired liver function.

Considering the drug options available for pain management, your doctor need to establish the minimum possible dose for you and simultaneously monitoring liver functions.

For sleep, ativan is liver safe as compared to klonopin.

Thank you for your response. I guess I miss typed my email address and just received this response.

What anxiety medications are renally excreated? Is ativan?

Also my potassium was slightly elevated.

Gabapentin gives me headaches.

ativan is safe in liver disorder.

The anti-anxiety drugs which are safe in liver disorder are lorazepam (ativan), oxazepam and temazepam.

You can safely take ativan even if your potassium levels were slightly elevated.

Gabapentin has this initial tendency of causing heaviness and pain in head.
It usually resolves in 7-8 days of taking drug.
However, if it does not suits you, you can avoid this.
