Question: My neurologist has prescribed pramipexole to treat my essential tremors. I started out taking .375, went to .75, then 1.125, then 1.5 and now I have been on 2.0 for 9 days. I have experienced balance issues, trouble walking and have been confused and disoriented a couple of time and could barely put a sentence together. I constantly feel wobbly and feel like I'm going to fall, I have fallen twice but had no injuries. He hasn't seen me for some time now and I'm under the care of his physician's assistant that is titrating me on the pramipexole. I felt the best when I took only 1.125 or maybe even the 1.5. What would you suggest?

I have read your question and understand your concerns.

Pramipexole dosage is increased gradually to target dosage 1,5 to 4,5 mg/day, divided three times a day, depending on symptoms control.

It is obvious that you need to decrease dosage to 1,5 mg/ day for two reasons:

1. you had sufficient symptoms control at that dosage

2. you are experiencing side effects.

So, I recommend to decrease the dosage at 1,5 mg/ day, divided in three times for a couple of weeks.

If no good symptoms control, or continue of side effects, after this time, then you need to discuss about other possible treatments with your Neurologist.
