What Causes Anal Itching, blood After Bowel Movement And Pain In Rectum?

Question: Been having pruritus ani on and off since August of last summer. I was examining myself 4 days ago and noticed the attached images (first 12 photos). Examined myself yesterday and this morning (last 7 images were from this morning) and noticed no change or more appearances of the attached. I am looking for a second opinion to possibly confirm what a previous STD specialist on healthcaremagic told me, which is that he did not believe this was a genital wart but more likely a hemorrhoid or possibly an anal polyp. Do you agree regarding the first 12 images?

Background:

Was examined and tested a month and a half ago by a STD specialist. In his examination he did not find anything he believed to be warts and the pap smear came back inconclusive for HPV. I also tested negative for all other STDs

I am a XXXXXXX man who engages in receptive anal sex 2 to 3 times a month

Always use protection (latex condom)

I have been taking for the last 8 months regularly (usually exceeding the recommended daily dosage) a raspberry ketone diet pill (http://www.walgreens.com/store/c/raspberry-slim-powered-with-raspberry-ketone/ID=prod0000-product)

I usually have 3 to 4 bowel movements a day and generally they are semi-solid and wet (already had 2 this morning)

I do notice after anal sex there are some tears in the anus

Symptoms:

Occasional anal itching around anus for last 8/9 months. Was using hydro-cortisone and that did help relieve the itching.
Occasional blood on toilet paper after 4/5 wipes (none this morning)
Occasional dull pain in the rectum and/or in buttocks
Occasional moisture or discharge sometimes before having to make a bowel movement. The moisture or discharge is not sticky, and is either clear or yellowish-orange/brown

DO NOT EXPERIENCE:

Constipation
Sensation of having to make a bowel movement
Feeling bowel movement is not finished
Feeling as if there is a lump or mass inside rectum
No growths around anus
No itching or growths near genitals

For your consideration in addition to my initial query:

Could the dull pain and blood be from tears in during anal sex?
Could the itching and moisture/discharge be from using (over using) the diet pill?

Thank you

Brief Answer:
biopsy required

Detailed Answer:
Hello
Thanks for the query and photograph. Pruritus ani is just a common term for peri anal itch.
By appearance it is looking like a wart.
I advice you to get it removed and a biopsy be sent.
Only biopsy can confirm whether it us wart or not.
Also excising the lesion will be curative.
Pain in rectum and bleed can be related to anal intercourse. However diet pills have no relation to your condition.
Let me know if you have any other doubts.
Thank you

The last doctor said by appearance and symptoms he thought it to be a hemmorrhoid, what leads you more to think a wart?

Brief Answer:
biopsy must

Detailed Answer:
Hello
Hemorrhoids are usually painful also they protrude out on straining and then decrease in size.
On the other hand warts are asymptomatic and protrude like a mass.
As I mentioned earlier the only definitive way to say that it is wart is by biopsy.
Also biopsy will remove the lesion.
Please meet a surgeon and get it removed.
I would like to know whether you feel any mass protruding out while defecation.?
Let me know if you have any other doubts.
Thank you

No mass.

I guess I am just confused because from everything i read and have been told warts more often than not appear in clusters and do not cause pain but in some cases itch. My main symptoms have been pain and itching and this is just one single growth.

Brief Answer:
details of both given below

Detailed Answer:
Hello
Please don't get confused.
I will try to explain both of the issues to you.

Warts can be single or multiple. It presents as a painless bump which has a surface showing very small finger like projections. The lesion bleeds readily on touch and remains if same size.

Hemorrhoids present as single or multiple lump with totally smooth surface. These are usually painful and the lump increase in size when you strain on defecation.
The photos sent by you are not that clear to see the lesion closely.
Please examine it once again and try to notice the points mentioned by me.
Thank you

No bleeding upon touch. I thinks its actually a hypertrophied anal papillae. Look at this article on hypertrophied anal papillae

http://www.ncbi.nlm.nih.gov/m/pubmed/0000/

aside from it looking like it, and it being on what appears the dentate line the symptoms match what I have said I have:

associated complaints like pruritus, pricking sensation, wetness, crawling sensation in the anus etc

I attached some more images to go along. The last is an online image of hypertrophied anal papillae that sort of matches my growth.

I do believe also my hypochondria exasperated the symptoms as today i feel much more relief and less severe symptoms having uncovered and researching

Thoughts?hypertrophied anal papillae

Brief Answer:
Looks like anal polyp

Detailed Answer:
Hello
Thanks for the details and photograph.
I checked the link and admire your research.
It looks like an anal polyp which can be easily removed by radiofrequency.
As you have history of anal sex, chances of polyp become more.
However still I would suggest that a histopathology of the tissue must be done.
We must not ignore even subtle things.
Let me know if You have any other doubts.
Thank you
