What Causes Ammonia Smell Coming From Testicles?

I am seeking further information about an issue my husband feels embarrassed to discuss with his doctor.
He is 31, 183cm high, weighs 102kg (large body frame) and non smoker. He works as a builder so on his feet and active. Over the past few years, an incredibly intense / offending ammonia smell is coming from his testicles, even straight after a shower. Changes in diet have not improved this.
He was a binge drinker for many years from the ages 18 to 27 and was diagnosed with a severe fatty liver and insulin resistance at the age 24 with improvements in his late 20's.
Are there any further tests he could do to seek the root of the problem and take the steps to fix this?
