Question: i am a very athletic 56 year old woman - just noticed a small bulge when i flex my leg at the top inside of my right calf muscle at the base of the back of the knee

This bulge can be a varicose vein or Bakers cyst.
Varicose vein is usually soft in touch, has vein like color and should be less visible when the leg is rise (when lying down for example).
Bakers cyst is also soft however can be painful. It can get smaller and larger especially after physical activity as this kind of cyst is connected with knee joint and filled with joint fluid.
The best way to find out what exactly it is, is an ultrasound.
In both cases surgical removal should be considered.
