Brief Answer:
Both, underlying case and symptoms should be treated.

Detailed Answer:
Hello and thanks for using HCM.

I have read your question and understand your concerns.

Neuropathy symptoms get worse if the underlying cause is not controlled well. (diabetes, alcohol use, viral infection, low B12 vitamin levels, kidney and liver diseases)

First, you need to get diagnosed correctly about the cause of neuropathy.

If this is already known, treatment should aim to control the cause and to improve symptoms.

Since Gabapentin isn't working, other drugs such antidepressants (amytriptiline, fluoxetine) should be considered to be used.

Since these are prescription drugs, you should ask your primary care Doctor to prescribe one of them.

Hope you found the answer helpful.

Let me know if I can assist you further.

Greetings.
