Question: Previously healthy 31 y/o male, 130 lbs, 5'10". Six weeks ago I began experiencing urinary urgency--after peeing I felt like I still needed to go, and the sensation fills about 70% of my day. UTI and STI tests are negative. Then about 3 weeks ago I began having pain while passing stool. I found a lump on my sphincter, and believed it to be a hemorroid, about the size of a pea, soft and pink, however I do not have any of the risk factors for hemorroids (I eat plenty of fiber, am not constipated, don't have diarrhia, don't strain going to the bathroom, etc.). The lump has never bleed, only itched and had sharp pain while deficating and sitting. The lump has ceased to be painful after application of Preparation H, but hasn't gone away. I have ceased eating foods that could be possible irritants (spicy, acidic), but experienced no improvement in the urinary urgency. I have a generally healthy diet and eat many vegetables and lean meats. I don't smoke and rarely drink alcohol. I don't consume caffeine. I have no previous health problems.

1) There is no correlation of urinary symptoms with your present GI symptoms.
2) You might be suffering from anal fissure with small hemorrhoid as per your description.
3) Anal fissure is condition in which small linear tear develop on inner wall of lower third part of anal canal. Constipation, excessive straining, hemorrhoids are risk factors for development of fissure.
4) Common symptoms are burning pain around anus, occasional bleeding.
5) Treatment consist of medication to maintain stool consistency soft (stool softener or laxatives), anesthetic agent ointment for local application (to relieve pain), use of anti-inflammatory medication (to reduce swelling).
6) There is limited role of surgery in anal fissure and hemorrhoids. Indication for surgery in hemorrhoids are big hemorrhoids, bleeding. Indication for surgery in fissure is episodes of severe pain refractory to medicines, bleeding.
