What Causes PCOS, Dull Lower Abdominal Pain And Lower Back Pain?

Question: I am a 23 year old female with polycystic ovaries. I had bloods done by my gynecologist on 12th August, showing slightly high levels of testosterone and that I have ovulated. I have not had a period yet nearly 3 weeks on. I have done three pregnancy tests all negative. I have a dull ache in the middle of my lower abdomen, pain in my lower back like cramps. slightly loose stools and feeling sick. could I be pregnant?! Or is it a cyst?!

I appreciate your concern

Based on your description, its unlikely that you are pregnant as three weeks of secondary amenorrhea should be showing a positive pregnancy test if indeed pregnancy was the cause. Its likely to be a cyst. An ultrasound abdomen pelvis can be done to look for an ovarian cyst and other possibilities. Blood HCG levels can be requested to be more sure in ruling out pregnancy. At 5 weeks a gestational sac is usually detected by an ultrasound so you can go for the ultrasound to rule this possibility out of the way.

Hello again.

If on sonogram, its a functional cyst then 6 to 8 weeks of observation can be given with a repeat test to look for regression in size. A complex cyst or a of size greater than 7 cm would require surgical evaluation. The ache upon urination could also be associated pelvic pain with the cyst and can be a UTI, too. Urinalysis and culture sensitivity testing can also be advised and antibiotics can be started empirically.
