Question: I have XXXXXXX having pane in one testical for a week or so. It is enlarged inside and rather hard. What is causing this ?

The pain and swelling of testis you are experiencing is likely due to Epididymoorchitis, which is bacterial infection and inflammation of testis and surrounding tubes.
Clinical examination, urine analysis and culture can confirm the diagnosis. I would also recommend an Ultrasound evaluation of scrotum to be done to rule out any associated masses/tumors of testis.
Management is with antibiotic like Levofloxacin, which needs a prescription.
Please be advised to follow up with your primary care provider for further evaluation and management.
Meanwhile, you can take Advil 400 mg up to 4 times daily to reduce the pain and inflammation.
