What Can We Do To Improve Chances Of Natural Pregnancy And Avoid IVF As IUI Has Failed ?

Question: Dear doctors,
My wife has PCOD, I'm 35, she's 29. We are trying to conceive but unable to so far by natural methods. Hence we've approached an infertility specialist Dr. Vidhyamani (Lalbagh Nursing Home) and have gone thru a few cycles of IUI but in vain. Pls advice on whether we stand a change at natural pregnancy and can avoid IVF. What can we do better to improve our chances at natural pregnancy. My wife is 64 kgs (5' 4") , i'm 75 kgs (5' 7") and are healthy otherwise. I'd also like to know if there is any specific food / diet that we need to incorporate in our diets to ensure a better chance. My sperm count is fine. I also masturbate once a day, we have sex about 3 times a week, also consume VITA-X Gold 2-3 times a week (to ensure better chances), does this affect our chances ?

I Would like to know few more things which would help me in suggesting you better:

1.) Since how long are you both married?
2.) Since how long are you trying for a Baby?
3.) Has she gained weight recently?
4.) Are her Periods regular?

In your Wife's case, PCOD is a condition where in numerous small fluid filled cysts will be located along the edge of each ovary, which affects release of Egg (Ovulation) causing difficulty in getting Pregnant.

Your Wife should Just follow the treatment Plan as directed by her Doctor and see to it that she goes for regular follow up.

Kindly have patience, since your diet which should include fresh fruits and XXXXXXX leafy vegetables.
2.) She should exercise regularly for an hour and maintain a healthy weight. Avoid high calorie foods
3.) She should avoid stress and practice relaxation techniques like yoga and meditation
4.) You should be avoiding smoking and alcohol
5.) Get a second Opinion from another Gynecologist before going for IVF
6.) Have alternate day unprotected sex between the 10th and 20th day of her cycle.

Thanks much for getting back Dr.Tauseef. Really appreciate this quick help and prompt & useful answer/advice as well.
I had forgotten to mention that we've been married for 7 yrs now and she was on contraceptive called Dianne for first 4 yrs (to keep weight in check and to support PCOS condition... hirsuitism etc. too). 5th yr I was not in town, was out on work. Its only since last 2 yrs that we've been trying to have a baby & went about treatment at Gunasheela Nursing home for a year and now since a yr at Lalbagh Nursing home(where she has shown improvement says our doc - was able to achieve fairly correct size of follicle for 2 cycles of IUI, both of which of course did not work after insemination too:(). As to her weight, she's more or less maintained it well but her periods are not regular, in fact she has heavy periods once in 4 months (if not on medication).. has to take off frm work for 2 days atleast. Since a yr, she's constantly on 1500mg metformin per day (for insulin resistance), Himalaya Evecare (1 per day) & WellWoman(Vit. supplement).

Follow up Q1: When u said : Have alternate day unprotected sex between the 10th and 20th day of her cycle - im confused since she has no cycle and as i said unless induced by medication (by gyno) her periods happen only once every 4 months.. and sometimes even that is not regular. So when do we have sex ideally, is my qn? More so since both of us are working its becoming exceedingly tough to plan our sessions too.. only weekends being our respite, thx to extreme stress levels at work... only today ive taken off frm off.
Follow up Q2: I like to masturbate(alone) atleast 1c every 2-3 days. Is this fine? Do I need to abstain from it on certain days specifically? Also are there any positions that aid pregnancy & does the force of ejaculation inside the vagina matter?

Follow up Q3: Sorry for bombarding u with more follow up qns.. here's the last one (hopefully!) - I indulge in alcohol(about 1c or twice a month - beer/vodka) with friends & smoke 2-3 cigarettes per week on an avg.. during my work week, abstain from both on weekends. I also indulge in meat(chicken, fish only) about once a week during my work week. Is this fine? Otherwise, I walk for about 15 min every day morning and have started going to the gym since a week, no other physical activity. Is there anyway (e.g.:Medicines like vita-x gold) I can increase the no. of times i have sex with my wife with this kind of life style so that our chances of conception increases? Im on 20 mg prodep, 1 wellman (vit tab), 2 Himalaya garlic pills everyday (to handle traces of cholesterol), no sugar in my coffee (as diabetes runs in the family & the last sugar reading a month back had shown 140 post lunch)

Thank you for writing Back.

You should be having alternate day unprotected Sex, once her periods are regular. You should be doing a Follicular study to see if she has released egg after Injecting Ovulation drugs, her treating Gynecologist will be able to explain it to you better.

You can masturbate only when you are sexually aroused, may be twice or thrice a week. For increasing the chances of conception don't masturbate, 48 hours before having sex with your wife.

Also adopt a Position which deposits the sperms much close to the cervix like Missionary Position (Man on Top), but there are no studies backing this. You can adopt this position during every session. Plan your sex sessions thrice a week, not just on weekends.

Avoid smoking and Alcoholism until your Wife conceives at least as they will affect your Sperm Quality.

Vita X Gold may not increase your Sex sessions but improve sperm quality.

Increase your Work Outs to an Hour every day, 5 days a week.

Ask you wife to follow the Lifestyle changes as i had suggested earlier, stick to her treatment plan and go for regular follow up.

Thanks very much for getting back with your valuable inputs Doctor! I have one last query which i had missed (last one for sure :)) :

My wife has undergone Ovarian drilling twice already in the last 2 yrs.. can she or would it be safe to go for one more time (we're yet to consult our gyno/specialist on this as we are still contemplating natural ways of conception rather than IVF.. we've kept IVF as the last resort). Thanks for all your replies sir, really appreciate all your assistance on my problem. It helped!

Ovarian Drilling or Ovarian Diathermy as we call it is not commonly done but it is an option tried in women with PCOS who still are not Ovulating after taking Fertility Medicines. Destroying part of the ovaries has been reported to restore regular ovulation cycles.

You can speak to your Doctor and get it done one more time. Studies have shown that Ovarian Drilling has resulted in 50% chances in conceiving normally.
