Question: Dr Lohit K please continiu conversation, I saw my doctor at one today, he said discontinue both drugs, we talked about, do nothing about the urinal infection,there's is no more he can do. I told him about ofloxcillin, said he has to go with a special list on my blood work only those mentioned, about 20 of them, your thoughts? What will happened to me about the infection , I have no symtems yet???

Be relaxed. Since there are no symptoms of infection no need to worry. Usually urinary tract infection (UTI) presents with symptoms.

Yes its good advise to stop the drugs that may cause allergic manifestation.

For UTI based on the patient profile there will be a list of medications that can be used. These will be safer compared to other general drugs. So among them are penecillins and cephalosporins. But among the list of drugs some may not act on the particular bacteria causing infection due to resistance and hence culture and sensitivity will be done to select the drugs. This is the procedure of selecting the drugs (For your information).

Now since you don't have any symptoms of UTI no need to worry, but follow general measures like
1. Plenty of oral fluids. This will help in flushing out the bacterial colonies or any debris at infected area.
2.Maintain personal hygiene. Use sun dried cotton inner wears. Maintain hygiene after urinating or defecating.
3. You should wipe from front to back (not back to front) after bowel movements. This helps prevent bacteria from the anus entering the urethra.
4. Empty the bladder regularly and completely.
5. Cranberry juice, has been shown to help prevent urinary tract infections. So you can have it.

These are the preventive measures that will help.

Dr.LOHIT, I have just read your precautions,and will do, I should also tell you that I have a enormous panaculus that is probably the reason I have these infections, I try to keep clean,shower every day,but still have this infection. Thought I would lighten u up,in my case ,at my age 81 I have all the goodies like diabetes with two kinds of insulin and Victoza. Thanks for your good judgement I will live a lil longer,.

Along with the measures explained, you should have good control of your blood sugar. The raised blood sugar is the main culprit of infections. Have balanced diet and take your medications regularly at right time.

Hello DR.LOHIT K I saw an ONCOLOGIST to day for a Melonoma stage one or two, he is a surgeon also, it is on my scald ,he told me it would be a difficult surgery because I am not a good candidate having all the different sicknesses I have, he said I would have to get rid of my infection first, than it would be hard to operate because there is not enough skin to close the wound, it would have to graft from another part of my body by a plastic surgeon,in a hostpital, should I get a second opinion.or talk to my private Doctor, I know you are not a oncologist maybe out of your feald, I am thinking maybe I should do nothing, he said it would be a difficult operation in a hostpital,stop my xperelto a few days, I value your opinion, but you do not know my whole health history, about that urin infection I have to go and give another sample or test tomoro to see if I still have an infection,thanks again you have been a good Dr,to me.

Hi,

No need to worry regarding the infection as you don't have any clinical features now. Good that you are getting retested for any infection.

I can understand at your age and comorbidities its very difficult for any surgery. But since its melanoma better to get second advise and you should keep watch on the lesion.

Before surgery we need to stop any anticoagulant and hence your doctor might have decided to stop Xperelto for few days as if he decide to operate then there wont be much bleeding complication.
