What Are The Symptoms Of Strangulated Hernias?

Question: I was diagnosed with a left inginal (?) hernia in 2008 and was scheduled for a hernia repair operation in August of that year. Unfortunately my cardiologist at the time would not XXXXXXX cardiac clearance for the operation so I have had to suffer with a growing hernia in my testicular sac since. My current cardiologist also will not XXXXXXX cardiac clearance for general anasthesia, but indicated local anasthesia may be possible. In the past two weeks I have experienced pain in my lower abdomen area with a sense partial strangulation may be occuring as bulges occur and gentle massage seems to alleviate swellings after some 30 minutes. My questions are as follows:
1. What are the signs of strangulation and do I have it if I still have regular bowel movements.
2. What kind of local anesthia is available for my condition.
I believe the cardiac clearance was not given for general anesthesia because of an abnormal ekg

I went through your query and understood your concerns.

As size of hernia increases, first obstruction occurs, in which bowel in hernia gets blocked and presents as nausea, vomiting, sudden indent of pain abdomen and inability to pass gas or motion and size of hernia is not reducible. In case of partial obstruction of bowel wall loose stool is a presenting symptom. If this obstruction is not corrected and still progression leads to reduced blood supply to bowel wall and death of part of bowel wall which is there in hernial sac.
Based on your description, I don't think it's a feature of strangulation yet. If the pain has increased visit your surgeon for clinical evaluation.

If patients general health is not good , then usually we prefer local or regional anaesthesia , in which anaesthesia given to spine at lower level, part operated will become numb but patient will be awake through out procedure.

As your hernial size increasing and pain( intermittent pain could be due to increased bowel movement or continuous dull pain due to dragging sensation of bowel wall)
Better to get operated under regional spinal anaesthesia.
