What Are The Side Effects Of Methotrexate?

Question: Hi,
I have been prescribed methotrexate for the treatment of psoriatic arthritis (20 mg per week, tablet form). My rheumatologist has said it is ok for me to have around 6 drinks per week total on the weekend, and he is going to monitor my liver closely. I was too embarassed to ask my rheumatologist but can you please advise what the interactions would be if any between methotrexate and very small quantities of recreational drugs including:
Marijuana and cocaine (not used together). I do not ever take large quantities of these and understand they carry their own risks but if you could specifically refer to any interactions with methotrexate i would appreciate it. Thanks for your help.

Normally we dont recommend alchohol with methotrexate as methotrexate is metabolized in the liver as well. So both of them combined can increase the liver enzymes.

As for Marijuana and cocaine, Considering your disease I would recommend you to go off it as smoking is the only known factor known to accelerate the psoriatic arthritis. It doesnt interact with the liver as such, but can increase the pace of the disease.

As for alchohol, I would suggest you can always take it once a in while, but dont take it on the day you take methotrexate. And try to avoid hard liquor. Stick with beer and wine. So you can take your methotrexate on a weekday( Monday/tuesday sorts) , so the effect will go down by the weekend.

But do monitor your Liver function tests. Any rise will be an indication to go off alchohol

Well, not really, they dont have any interaction but every drug gets metabolized in the liver( CYP 450 enzyme), so who will interact with who is always a toughie.
