Question: I visited a transgender sex worker 1.5 month back and had a anal sex with condom. My symptoms started a month back when i saw a cyst develop on my penis and it was small cyst. It had skin color. Later after couple of days, it grew larger and color changed to red. For same i went to dermatologist and she gave me Allegra with a steroid based cream to apply. The cyst started to subdue. In meanwhile, i started feeling pain in head of penis and little tingling sensation. After 2 weeks on visiting dermatologist, i felt little burning sensation in my left testicle and it was red. So i again visited the same dermatologist and she said that i may have fungal infection when i told her my history of fungal disease. Later after 3-4 days, i started having pain in right testicle occasionally, not constantly. I was worried and at same time, i felt burning sensation while doing stool. I also felt back pain, chest pain and little head sometimes. These symptoms were not constant and were coming and going. For same, i went to urologist and he gave me test for STD's, blood, urine, semen culture. He also gave me doxy tab 100mg alongwith Alfoo10mg, Disperzyme and Ultranuron. He Suggested semen analysis. I have started the medicine but my symptoms are still coming. After started medication, i started feeling pain in my lower calves of both the legs. After a day, it stopped paining in lower calves but i felt mild pain around my right knee and in my right foot and heel. Also I now got loose motion which i checked with my urologist and he said that may be Doxy tab is causing it. So he stopped it and started Ornof. In meanwhile, yesterday i felt mild headache which was constant alongwith flu like symptoms like sore throat, fever of 99.F and runny nose. Flu symptoms went away but head ache remain till the time i slept. Today when i woke up, headache was not there but i felt pain in chest and throat. Today i got news from lab that my Test for gonorrhea and Chlamydia are delayed by 10 days. I was waiting for them since my other test are negative or normal. I am worried as i asked doctor and he told me to wait for the test. I am confused and worried specially i checked net and it shows that my symptoms are similar to gonorrhea. I am planning to tell my family about it and ask doctor to start medication without test. Otherwise i fear, disease may end up hurting my body more.
I also had burning sensation in my lower abdomen which have gone since i have started antacid and other medication. I am confused if it is also sign of the gonorrhea or just due to acidity.

Brief Answer:
I suggest test for genital herpes; Your risk of acquiring gonorrhea is nil

Detailed Answer:
Hello. Thank you for writing to us.

I have gone through your query and I have noted down your concern. I have also reviewed the attachments.

I don't think your symptoms are due to gonorrhea.
Gonorrhea in males usually presents as frankly purulent urethral discharge and burning micturation and pain on passing urine.
Moreover since you sexual act was a protected one so actually there is no risk of acquiring gonorrhea from that particular sexual encounter.
As far as the risk of acquiring other STDs from your recent protected sexual encounter with transgender is concerned, it is only for genital herpes and genital warts and therefore I suggest you to test for genital herpes.
Your test for gonorrhea or chlamydia most likely would turn out to be negative simply because your sexual contact was condom protected.
Your symptoms are non-specific and are not related to STD, and therefore I suggest that you visit a physician for all these symptoms which seems to be related to something else rather than due to this particular sexual encounter. Some of your symptoms also seem to be related to anxiety.

Can you suggest the test for it? I will visit a lab and get myself tested for herpes and genital warts.

Also there was unprotected oral sex involved which i missed to write but anal sex was protected

Brief Answer:
ELISA for Hsv type 1& 2 (IgG and IgM)

Detailed Answer:
Hi.

You may test for genital herpes by an ELISA test for Hsv type 1 & 2 (IgM and IgG). Oral sex too carries a risk of herpes but it is not a common risk factor as far as the other STDs are concerned.
