What Are The Effects Of Severe Psoriasis?

Question: Dear Doctor, One of my friend has received a marriage proposal from a psoriasis patient.He is currently suffering from Psoriasis and psoriasis arthritis also his liver enzymes has also increased beyond the normal range.Wanted to understand if he will be able to live a normal life and what are the risks involved.

i am dr.kakkar(dermatologist). I have gone throuh your query and i have noted your concern

In majority of patients with psoriasis the disease is limited in extent i.e a few patches with or without scalp involvement. However in a small percentage it may progress to widespread body surface srea involvement and may be accompanied with psoriatic arthritis and in these patients systemic medications may be required. Obviously widespread involvement and joint involvement would impact on quality of life as compared to when only a few, limited patches of psoriasis are present. However, having said that there are numerous treatment options for widespread psoriasis and odoriatic arthritis like methotrexae, cyclosporin, adalimumab(humira), retinoids, phototherapy etc and ultimately the disease can be effectively controlled.
Liver enzymes can be elevated in patients of psoriasis who are on certain medications like methotrexate i.e it is a drug side effect Or else raised enzymes could be due to a concomitant condition like hepatitis, alcohol intake etc. Since psoriasis can vary in its presentation from time to time and from patient to patient therefore it is difficult to expect how severe the disease would be in future i.e one cannot predict its future course. It may remain localised or it may spread to a larger body surface. Treatment is guided by extent of involvement.

Hi Doctor,
Currently the person is getting good results using ointment for psoriasis treatment. Affected areas are elbows, scalp , back and Knees. During biopsy he came to know about the increase in liver enzymes that is beyond the normal range due to this reason Rheumatologists is not able to give him medication for psoriasis arthritis.Wanted to understand what are the other health issues that can happen because of this.

Certainly raised liver enzymes/ abnormal liver function can preclude systemic medications/ treatment for psoriasis with methotrexate which is used to effectively treat psoriasis and psoriatic arthritis.
Is the patient Obese?Or has Diabetes? Or consumes alcohol? Or has hepatitis? All these factors can also raise liver enzymes
Psoriasis patients may also have a higher risk of cardiovascular disease as compared to the general population.
