What Are The Chances Of STDs After The Protected Intercourse?

Question: Hi,
Good evening Sir/Madam
My quiery for HIV infection chances with STD,
Also about HIV window period I had protected sex with call girl on dated 3rd Feb 2014, but I'm already suffer with STD(Molluscum Contagiosum) and it is on my penis length ending point and testis,is there is any chance of HIV infection. On 3rd Feb 2014 to till dated when I'm tested HIV test on 15 week(20 May 2014) I'm not engagd any other sexual activities.
I had HIV test (ELISA) after 2,4,10,12 & 15 week intervals all are shows negative results (HIV antibody comb test,rapid test and also after 12 week I had testing with HIV antibody screening test includes p24 antigen from Dr. Lal Path labs,New Dehli it is also negative) and also after 15 weeks all are negative, Is there are any chances of HIV infection after this period. After this period it is consider as a conclusive result or not and I m feeling all the symptoms of HIV aute infection like as joint pain on back side of my lower body(back), joint pain on neck, flu, hedeque, body rashes,swallon lymph gland very small size like as half of peanut on right side of neck, pain in testis and lower stomach. Body rashes appears firstly on one right leg thais, it look like a pimple small red and little ichy and after some day(4-7) its goes black dot and than disappear & some rashes on my arms look like differently than leg thais rashes its look like mosquito's biting and appears on warm weathers conditions(30-33 degrees temperature) and disappears with in some hours, and also some strain on eyes and some time it is little readish also some little red dots(like as a pen dots) on my inner side of penis head after 2 months from sex.
What is the conclusive window period according to NACO and WHO' it is different according to country's my native place is XXXXXXX what is the window period in XXXXXXX
Is ELISA test is 4th generation test.
Please tell me all my symptoms are indication of another type of STD's kindly give me a details of these type of STD's which have same symptoms like as a my symptoms. I'm also tested with vdrl which is negative. Please give me details of other type of std,s test which I can be tested now.
Please HELP me,I'm worry about this because my marriage came this year,so I'm not distroy anyone life because of my blunder mistake, also I'm not concentrate on my work and my personal life.

I can understand your concern for possibility of HIV or any sexually transmitted disease after the protected intercourse with commercial sex worker.

I have thoroughly read the complete query and after analysis of the query with diligence let me first and foremost reassure you that you do not have anything related to HIV. Neither your tests show any symptoms of HIV and nor does your symptoms too.

So first of all just relax and take a deep breath of confidence that you are free form HIV.

Let em give you the valid explanation for the same:

First of all your contact was protected. I have never seen in my clinical practice to contract HIV by protected intercourse. Though there are reports of HIV even after unprotected intercourse but are beyond our discussion because of your negative status.

Secondly we do not know the status of the girl. Let us consider that she was HIV positive. In an unprotected intercourse with a HIV positive person the HIV viruses gets entered into the body and divides in in numerous numbers(millions of viruses per ml).

After a period of 2-6 week there is mounting of antibodies against the viruses. These antibodies can take at the maximum of 8-10 weeks of the period to get expressed.

There fore the infection can be analyzed by two ways -

-A): By HIV virus detection: done by RNA- PCR. It tells us the number of copies of virus present in per ml of blood. By the latest of methods the viruses can be detected within 2 weeks of period.

B): By HIV antibody Detection: As told earlier once the antibodies appear maximum of 8-10 weeks then these antibodies to HIV virus persist throughout life.

The protocol of the antibody tests done for HIV is as follows: One HIV antibody test at 6-8 weeks and then a follow up final test at 3 months (84 days or 12 weeks).

Antibodies once formed to HIV can never disappear. So if a person has HIV infection, 3 months of period is the time when we say to be fully sure of whether HIV is present or not.

Now you must be worried about window period. To tell you simply about window period- it is the period of entrance of the virus to the detection. Now it depends on the various tests to detect the viruses. In some test it may be up to 6 months but in latest of all the tests done including HIV test (ELISA) with p24 is the latest one which can detect within 6 weeks of period. But for all the standard operating protocols we consider 3 months as standard period for all the cases. So do not get confused with the window period given at different sites or references. Your test at 12 weeks is sufficient to declare your negative status and you do not require any further testing.

Regarding your other of the symptoms - though as I cleared all your doubts regarding HIV but even then you should know certain more points. This quite common to find one or other symptoms which you enumerated in acute anxiety states. You can very well notice that there is no fever (of high grade) in your case which is one of the most common symptoms of HIV.

I request you to kindly do not read by your own on internet anything related to HIV.

Firstly thank you very much Dr. For your quick and motivating reply.
But my quiery about sex is protected with condom and condom protect my penis,but I have Molluscum warts on my penis length ending point and testis which are not covered by condom,so my anaxity about for this issue.
I'm not feels this type of symptoms not single one before this incident(sexual exposure),so I'm worried for this.
Please tell me this is any type of std's infections,give me a details of these tests for digonsis for these std's.
Is these symptoms like as other std's infections can I go for other std's tests.
Can I go for further test for PCR RNA test for detection of virus.
Is it accurate after 15 week of sexual exposure.
Is there are availability of any test which are give accurate result after 15 week of exposure.
All are my HIV test occur in ICTC centers and one is( HIV 1 & 2 screening comb test including p24 antigen) done in Dr. Lal Path Lab, Collection Center, Shimla,H.P and all are results gave me next day there is any chances of false result due to delay to next day.
I'm suffer with T.B when I'm 15 year old,so there is any chances of these symptoms related with my past T.B infection.
Due to molluscum warts how many percentage of HIV and other std's infections.
Kindly help me..

Hello young fellow,

Welcome again to the forum and thanks for asking further queries.

A negative test for HI means negative and there is no doubt about it. You know the mater of HIV is so sensitive that whenever a report is prepared then utmost care is taken to interpret as well as handing over the report to the person. ICTC center as well as all the private laboratories have all the latest and advanced tests to give you the result.

There is no effect of your previous illness or any other thing on the test report. All the tests and especially those done at 12 weeks and after 15 weeks are confirmatory to give your clear status.

Regarding molluscum contagiosum- in these cases the skin is still been intact so there is no chance of entrance of HIV virus through the intact skin. You probably know that HIV do not spread by skin to skin contact. That is why your reports are negative for HIV.

IN my clinical experience there is no need to go for RNA PCR for HIV even if you want to go for then it would be cent percent negative and therefore will relieve all your tensions for HIV.

I many a times encounter your kind of cases where in anxious states they start reading the symptoms of HIV and start correlating with there own. Stress is such a big thing that person himself do not know when and how they start feeling the same symptoms as they read.

I am not saying all these above thing just to relax you but based on the finding of your tests. In HIV testings timing of testing is quite important. Had you been within 6 -12 weeks of exposure then I would have guided for RNA PCR or ELISA at 12 weeks. But as more than 15 weeks have already elapsed and the antibody tests are negative so just relax about yourself.

To tell you most importantly that in my own clinical experience I have never seen any case negative at 8 weeks and turning positive later on any time. So even the best limit is 12 weeks according to guidelines but in my experience even 8 weeks is suffice.

I can also understand your part of anxiety as a young man to be getting married and therefore understand the seriousness of matter involved with two lives. So believe in me and start thinking other than HIV.

Talk to yourself and say that you are clear. Boost your moral and do not feel guilt for the normal desire accomplished. It is a thing of past now and there is no correlation with present state.

Good evening Dr.
Once again,I'm here..
Today I had done HIV Rapid test of these girl(sex worker),in which I had sexual exposure in last 16 week before, the result is non reactive of this girl,so now their is any worry about my infection. She is also 16 week of exposure with me.
If there is no worry about HIV infection than why I'm feeling these type of symptoms of aute HIV infection.
You are darmatalogiht specialist so kindly tell me why I have a skin rashes( small red,little ichy pimples) on my thai and another different type of rashes on my left arm just one and two pimples little ichy and these are disappears with in one and two hour and it is totally different from thai rages.
Why i,'m feeling swallon and gagging in my throat as well as joint neck pain and one side of testis pain little much sometime..
Today result of girl is consider conclusive for non reactive on my side..
What are your suggestion about other type of std's infections in my case and what types of tests you are suggest me..

So you got it right that if the girl is negative so your status is accordingly clear and negative.

I think you are out of the aura of HIV and its related things. So now you also finally believe that you do not have HIV virus in yourself.

For the further part of queries regarding skin rashes and rest of other things- First foremost you clear your mind regarding any of the possible sexually transmitted infections.

As I guided you for HIV, in the same manner I guide you that you are clear from any other of minor or major sexually transmitted infections after the protected intercourse.

None of the sexually transmitted infections manifest in this manner which you are experiencing.

Regarding the skin rash - there are thousands of reason of skin rash. HIV or any other sexually transmitted infections are not the common cause of it. As per your description it appears to be simple urticaria or commonly known as hives. The rash in urticaria persist for few hours and then disappears by its own.

IN my patients with such kind of problem I generally prefer to give a simple antihistamine like loratadine for one to two times daily.

The symptoms of gagging and swollen throat can be related to urticaria or can be due to simple anxiety states.

The same anxiety can cause perception of joint pain as well different sort of pains including testicular pain too.

So I again reassure you that you do not have either HIV or any other sexually transmitted disease. There is no testing required any further and therefore relax completely.
