Question: Hi Doc,
I went to a thai soapy massge parlor. I penetrated the girl's vagina very slowly may be 10 times whilst wearing a condom. I am sure even my base did not touch her genetal area. Thereafter whilst the condom was on, she did a handjob and oral on me. My worry is vaginal fluid on the condom after protected penetration could have massged in to my base when she performed the hand job or the oral. Is there any STD risk?

It is unlikely that the vaginal fluid in condom will have entered you. Condoms fit tightly over the erect penis and the penis head is the only probable route of entry of any infection in males. Therefore the question of vaginal fluid entering through the penis does not arise in your case. Even if a drop or two trickle to the base of penis during oral or handjob even then it will eventually dry out and is unable to enter the penis opening.

STD risk is almost nil as long she did not have any open sores on herself (genital and mouth) and you also did not have any open wounds in the genital region.

My worry is not that. I am not sure if the vaginal fluid on the surface of the condom touched her hand when she started the handjob and then massaged in to the base of my penis when she was doing it hard. What is the probabilty of such a scenario?

And my concern is not HIV, but skin to skin STDs

Probably the last point from me; do i need to get myself tested?

Certainly, HIV is not a risk for you as you have taken all precautions. I wanted to detail about skin to skin infection. In this group we have Herpes and genital warts as the two conditions that are highly transmissible. That is the reason I mentioned above about the presence of sores or rashes in her genital region or mouth and any open wound in your genital region. If there was no such appearance then you are safe.

If you are still having doubt then you can get antibody testing done for Herpes 1 and II and Human papilloma virus (causing genital warts) after 3 months. Though I assure that you should be testing negative if there was no active disease or rashes on her.

Your question "My worry is not that. No she did not remove the condom. It was there throughout the oral and the handjob. She applied some cream also on the condom before the handjob and the oral.
Will that change your answer? I did not see sores.
Do you still think that i need to get tested?

It is unlikely that you are in danger of any STD if the condom was there all along and she did not have any sores. In this case there is no compulsion to get tested. However getting tested will always clear your mind and give you confidence that you do not have any infection.

If you feel that getting tested will make you feel better then you can go for it after 3 months. This is because after three months, most sexually transmitted infections cause antibodies to be circulating and this allows detection. Please remember that it is your choice but in the end when test results will be negative, you will be happy and at peace of mind.

On the other hand, if you choose not to get tested after 3 months then there will be constant thought of uncertainty and that will disturb your daily life. So better you get tested and be happy with test report as proof.

Apologies for abruptly ending the convesation even for a short period.
I think I have not communicated the issue, may be.
I fail to understand as to why you refer to her sores. My problem was whether any vaginal fluid that she contacted from the surface of the condom (after penetration) was massaged into my base of the penis when she was performing handjob or oral on me with the same condom on. My beleif is even without sores, the vaginal fluid when massaged on the skin could lead to STDs. Also, it may be important to know as to how long the STD viruses could survive outside the body even in a moist environment (on the condom surface).

1. In any disease there is something called symptomatic transmission and aymptomatic transmission. When we talks of herpes, symptomatic transmission is much higher than asymptomatic transmission. That means if she is having sores then the transmission risk is much higher than if she is not having it. It also implies that the viral load or concentration of viruses in the body fluids including that of vagina is higher when a patient is having sores and rashes. In the absence of sores the viral load goes down and therefor the risk of transmission of infection.

2. The risk of direct transmission in a person without symptoms is 5 percent and that in a patient with symptoms approaches over 90 percent. That is the reason I asked if she had sores or not. I hope you have understood the reason for me asking that question.

3. The herpes viruses get destroyed in air after about 10 seconds. In moist surface it might survive for few minutes. You have written that she used some cream on the condom. Its possible that she also took precaution and used an antiseptic to clear the vaginal fluid.

4. Your belief that even without sores if vaginal fluid comes in contact with your penis and transmission of Herpes. In the absence of sores this is only 5 percent possible and even for that the vaginal fluid must contain sufficient virus concentration (unlikely if she does not have sores) and you must have a source of entry for the virus like a wound.

5. Many people might have the condition like herpes but will never get the rashes or sores. So they even do not know that they have herpes without doing the test.
