What Are The Chances Of HIV Transmission Through Non Penetrative Sexual Activity?

Question: Dear Dr.
My question is simple, but I can't wait to hear from a professional about it. This happened 48 hours ago

I went to a stripclub. I am 29, m, straight guy.
The girl was old around 45+. She gave me lap-dance. I was fully dressed (boxer+jean) during whole period while she was nude. While performing lap-dance over me, I liked/sucked her nipples, I did not feel any fluid/milk/anything else coming from it, but not 100% sure because at that time I was not very aware.

There was no sexual intercourse. Later I was nude and the girl was nude too our genitals touched may 5- 10 sec (on her anus and vigina) 3 to 5 times

I was very much concerned about std, however, now after searching the forums I am scared. My knowledge was hiv transmit from (1) unprotected sex (2) sharing needles/drugs (3) mother to child.

Questions:
1- Is sucking a woman 's nipple by a man and if some fluid was sucked will HIV transmit?
2- Should I test, If I suck (no to very little fluid, as far as I remembered) for very small interactions (4-5 times consisting of 5-15 sec) for any std/hiv? (the nipples seems to be al-right, but again not 100% sure)
3- My penis touched and rubbed her thighs, her vigina and her anus for very small interaction (unprotected) (4-5 times consisting of 5-10 secs) (no intercourse) Is it possible HIV could transmit and should I test? (not ware if there as any fluid on her vigina or anus but I am assuming I had percum) (I believe she had some fluid on her vagina)
4- The girl, seems to serving multiple customer before she came to me. I do not know her HIV status.

I need confirmation from you pls

1- To be safe do I start PEP?
2- Should I test for hiv/std?
3- Am I too concerned for some this that not an issue?

No need of PEP. Practically no chances of HIV.

I understand your concern and situation.

In your case there is practically no chances of HIV infection as there was no penetrative intercourse. I would suggest you not to worry. I would not suggest any post exposure prophylaxis. But yes for your mental satisfaction you can get tested for HIV by PCR after 5-6 weeks or 4 th generation ELISA test after12 weeks. But I assure you that it will be negative. As for other STD related diseases I would suggest if any new symptoms like itching, pain, lumps, discharge, etc develop you may visit physician and get tested.

Thank you for your advise and answer, I have some more follow up questions:

1- My penis touched and rubbed her thighs, her vagina and her anus for a very small interaction (unprotected) (4-5 times consisting of 5-10 secs) may be less (no intercourse) Is it still possible STD diseases could transmit just in 2-3 touches each less that 8 secs?

2- Should I test for STI or not required? If yes when should I test? (not ware if there was any fluid on her vigina or anus but I am assuming I had percum) (I believe she had some fluid on her vagina)

3- Is there anything I can do to reduce the infection like cleaning?or take antibacterial? Eat better? to prevent incase there was any STI involved?

Hi
There is extremely low chances of getting infection. You can take azithromycin for prophylaxis under doctors guidance. You can get tested by PCR for chlamydia and gonorrhea after 2 weeks post exposure. You can also get VDRL test done. Keep your general hygiene maintained. As such you cannot take special precautions now except observation or treatment for common STD if high risk exposure. Abstain from having intercourse with others till test results. Practice barrier methods during intercourse in future.

Sorry another follow up question.

I was sleeping last night and after an hour I suddenly woke up around 2am, and my face and neck was sweating. Is this a sign of HIV symptoms (I read this is the early stages symptoms)? It is almost 3 days after I encountered the club incident. I am really worried.

You must not worry as this is not a sign or symptoms of HIV. It could be related to REM phase of your sleep. You need not worry.

I went back to sleep woke up again around 5am and my hand was numb. I thought it was may be the way I slept but I just had to ask?
What should be my next step?

1. Should I stop worrying, and move on?
2. Should I Be careful and try to identify each and every symptoms?

I would suggest you to have patience. Do not think about each and every symptoms so much. This is normal and you must stop worrying about anything. Trust your doctor. If too much anxiety sedative like alprazolam will be beneficial.
