Ultrasound Showing Liver Cirrhosis. Endoscopy Showed Duodenal Ulcers. LFT Was Altered. Is It The Case Of Cirrhosis Or Just Fatty Liver?

Question: Hello,
I had altered LFT for quite some time. It was first detected in 2009. Since then it kept on going high n normal. In Feb, 2013, I had my ultrasound done which showed that I have got Liver Cirrhosis. I visited ILBS (speciality hospital for Liver) and had few tests done as follows:
1) LFT- Total billirubin as 2.0 and SGOT/SGPT as 39 each.
2) Ultrasound Report with mild coarsened echotexture
3) Fibroscan with value 6.8 kPa.
4) Few more tests regarding Hepatitis and some immune tests. All of those came ok.
5) Had endoscopy done which showed duadonal ulcers. Doctors prescribed few medicines for that but they didnt confirm if it is actually Cirrhosis.
6) Had my LFT done again in Apr and XXXXXXX again with LFT values normal.
7) I have been taking Evion 400 MG once a day.
8) Got my LFT done again in Oct 2013 and values came as :
Total Billi: 1.1
Direct: 0.6 ( normal range 0.5)
Indirect 0.5
Total Protien: 7.6
Albumin: 4.2
Globulin: 3.4
SGOT, SGPT: 30 & 38 respctively
Alkaline Phosphate: 254 ( higher than normal values)..

I would like to know if it is really the case of Cirrhosis or just the fatty liver? Please let me know if You need more details.

Hello! Thanks for putting your query in XXXXXXX I am a Gastroenterologist (DM).

Liver cirrhosis can be confirmed in your case by liver biopsy only. However what all you have mentioned it doesn't appears to be cirrhosis except for the report of ultrasound in Feb 2013 for which you haven't mentioned the findings of scan.
For ex: Serum albumin is normal, fibroscan value, Ultrasound abdomen in ILBS which showed only coarse echotexture and endoscopy examination.

Will you please tell the detail report of ultrasound in ILBS especially for portal vein diameter, spleen size. If the portal vein diameter and spleen size is normal then it is only fatty liver for which:
1. Continue with Capsule evion
2. Take ursodeoxycholic acid
3. Do reduce your weight if you are overweight or obese
4. Do regular exercise and take low fat diet
5. Get your lipid profile also
6. Stop alcohol intake if you are taking any
7. Get yourself vaccinated for Hepatitis B if not already done
