Brief Answer:
Increase dose.

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
In the case scenario of your father in law, I would give a re trial of giving oral anti Diabetic drugs and if still not controlled he would require insulin.
I would advice in such case :
1. Gliptins like Sitagliptin or Vildagliptin.
2. A combination of Pioglitazone, Glimiperide and Metformin.
Currently he is just on Glimiperide and Metformin as I could understand from his prescription attached.
In case these medicines also fails insulin is the only best drug for him.
Also he needs a good BP control and quitting smoking.
There is lot to do in his case scenario and if correctly managed he might not require insulin.
He needs to visit a doctor who specializes in Diabetes.
Share my opinion with him.
In case you live somewhere near XXXXXXX you can make him visit my clinic.
I hope it helps.
If you have any further query, please ask me.
