Suggest Ways To Manage The Side Effects Of Femilon

Posted on Wed, 8 Feb 2017
Question: I am 36 yr old ,my youngest kid is 8 yrs old, had undergone 2 c-sections.
I have stubborn acne on my chin.
Was on XXXXXXX for 3 yrs, stopped using it since 2013 since my lipids were increasing and had palpitations and hypertension, so the cardiologist asked me to stop immediately.
Cycle became normal after a few months.
In November 2016 got my tests done....t3 and t4 were normal but tsh was 7.8 so I went to endocrinologist, he suggested to go on aldactone for 2 months as that would help with acne and hypertension.
But after 2 months tsh is less than 0.01 and testosterone has increased from 42 to 63 now the doctor has prescribed nebistar 5 mg for bp and femilon for hormones.I have had side effects of XXXXXXX also earlier and now femilon.........don't know what to do

Brief Answer:
Consult a gynecologist and get a IUD implanted,Stop all oral contraceptives.

Detailed Answer:
Hi,
Firstly thanks for choosing health care magic for your query.

Have noted all your details,
A number of intra uterine contaceptive devices called IUD are available these days. They are implanted into the uterus and small amount of hormones are secreted locally into the uterus every day preventing implantation. A long term protection from pregnancy is achieved after that without any systemic side effects.

Birth control pills contains hormones estrogen and progestrone which have a number of side effects on body, involving deranged lipid profile,Hypertension,Acne and increased risk of cardiac disease.

I don't know why your endocrinologist have not suggested you intrauterine devices,When you are having all possible side effects of oral contraceptive pill.

Stop all medicines consult a gynecologist and get a IUD implanted, It will provide protection for many years without any systemic side effect. As the level of hormones become normal most of your problems will settle down.

Advice
Get a Intrauterine device implanted.

Thanks

Brief Answer:
Details below!

Detailed Answer:
Hi,

Your TSH earlier was 7.8 which indication of hypothyroidism. Throxine (Eltroxine) would have been started by your doctor though not mentioned by you. Aldactone has no role in the treatment of hypothyroidism unless you have missed some information. External thyroid in the form of medications - thyroxine is the treatment of choice.
It is important to know the cause of low TSH value. Most probably it would be due to the effects of medication - external thyroid. Stop those and the levels will be fine in a month .

Do you have polycystic ovarian disease?
Your doctor is treating you with birth control pills and aldactone which is the treatment of choice for polycystic ovarian disease. Person with polycytic ovarian disease have high level of testosterone and treatment of choice is already given to you. Stop femilon and go for iud. Excess intakes of hormonal pill are cause of problem in your case.

If you don't have PCOD in that case discontinue your birth control pills and level of androgens will be normal in 4 weeks.

Discontinue medications for for weeks ,and repeat thyroid profile and serum testosterone!

Continue nebistar for 4weeks!

Advice
Attach reports of USG abdomen!

Thanks

no Dr I don't have pcod ...after I stopped XXXXXXX on my cardiologist's suggestion in 2013 I got stubborn acne on chin....for which I consulted dermatologist who got my hormone profile done in august last year since my acne was agrevating during ovulation and periods that is twice every month( I have regular cycle)
but all hormones were normal and in range. so he gave me only topical treatment .
Then somebody suggested me that I should consult an endocrinologist if my acne is worsening during cycles.
But before consulting him I got all routine tests in November. All tests were normal except tsh which was 7.8 .
then the endocrinologist prescribed aldactone which could help in high bp and acne as well ( up till now I was not on any medicine except nebistar).
He asked me to get my labs done for tsh and testosterone and lft after 2 months.
Now my latest reports show tsh less than 0.01 and testosterone increased front 42 to 63 and deranged lft with increased sgot sgpt and alkaline phosphate.
Yesterday he said this is not possible with aldactone...and wants me to start with femilon to balance the hormones....

I just wanted treatment for Acne...neither my cycle was irregular nor I needed contraception solution....
Now I don't know what to do with this hormonal imbalance.
plz suggest...

Detailed Answer:
Hi
Tsh 0.01 doesn't makes any sense, most likely its a faulty lab reports,Repeat the test again.Tsh can never be as low as 0.01.
Aldosterone is a potassium sparing diuretic which has nothing to do with acne!

SGOT and SGPT raised to as high as 3 times of normal are considered medically deranged.deranged lftvvan also be indicator of cirrhosis of liver.a ultrasound should be done. A ultrasound liver should be done to rule out the cause of deranged LFT its the baseline test.

Continue femilon for 3 monnths to treat your hormonal imbalance.iIt contain estogen and progestrone and will supress the formation of testosterone and other androgens. It will take at least 3 month time.

Advice
usg liver
repeat Tsh
acne is mot treated with alsactone ,its treted with clindamycin phosphate and floroquinolo group of antibiotic and retinoic acid.
Follow up with endocrinologist with all reports

Thanks
