I have gone through your query. Initially you should rule out malabsorbtion syndromes and parasites. A stool routine examination will be helpful to rule out this. You can also do Deworming with anti helminthic drugs like Albendazole once a year.
Ideal weight is your Height in cm minus 100 plus or minus 5. You can gain weight by taking proteinaceous and balanced diet and doing mass building exercises. For mass gaining you can do body building exercises twice weekly. You can take foods like egg, meat , chicken, milk and milk products and fish which contain good quality protein. Taking nuts like almonds , cashews and peanut is also good. You should also include citrus fruits to get vitamin C and anti oxidants. Taking protein powder is also good. You can take protein powders without steroids like B protein powder with milk.
