Brief Answer:
follow the advises.. maintain a healthy lifestyle..

Detailed Answer:
Dear XXXXXXX Welcome back to HCM,

I've gone through your query and understand your concern..

I've reviewed the reports and your cholesterol is really quite high(511) along with elevated VLDL(a component of bad cholesterol) also (compared to your last month's reports)..

It's good that you've stopped paratha, but it's much more concerning than that..

So, at first just go through my questions and answer them quickly so that the whole picture gets bit clearer and it'll help to diagnose the most probable cause-

1. Have you ever got checked for Diabetes ? if yes then let me know when and how was last reports (the exact numbers of the parameters)..

2. Does anyone in your family have high cholesterol/any cardiac problem ?

3. Do you smoke/consume alcohol ? if yes, then you must STOP it IMMEDIATELY

4. You've already been advised for drinking more water, are you following it ?
You need to drink at least 3-3.5 Ltr of water per day..

5. Any pain abdomen/lethargy/discoloration of skin over any part of the body ?

6. Any other past/present medical concern ? feel free to share..

7. Are you on any particular diet/supplements ?

8. Have you visited any GP/Cardiologist yet ? if yes then when and what was advised ?

9. Even though you've stopped eating paratha but still there are a list of other items which can prolong it over a period of time -

excessive consumption of carbohydrate(sp. more than 60% of your diet)..

junk foods in other form i.e. pre-packaged fast foods/processed fast foods i.e. pizza,burger,hot-dogs etc.

excessive consumption of packaged fruit juices and other synthetic and artificial flavored drinks and carbonated beverages(all those fizzy, carbonated beverages)..

So if your still consuming any of those above, then you need to cut them down..

Start having whole grain products instead of refined flour produces..

Please get in touch with a dietitian for a complete diet chart based on your BMI, current lab parameters, daily activities to get the proper benefit out of these modifications..

Exercise is a must and you need at least 30-45 min of physical activities and work-outs, so start implementing if not started already..

Let me know the answers of all those above mentioned queries, so that I can advice you on the further steps of management..

Awaiting for your response..

Take Care
Kind Regards

1) Never checked for diabetes. But my father has diabetes. I had few months back gone for routine health checkup. attaching report.

2) My father had minor heart attack few years back. No one else has any cholesterol or cardiac issue.

3) No, I neither smoke nor drink alcohol.

4) I drink around 2 litre water everyday. will increase water consumption as advised.

5) Currently no pain or issue. It was some health checkup in office 2 months back when I first reported high cholesterol (total 311). So I changed diet. Started Daliya (salt) in morning, stopped any oily item, stopped outside food, stopped eating
eggs , drink daily 250 gm Chaach instead of milk.

6) Around 5 years back I had stone problem in kidney. No other health issue reported except that.

7) No supplement, except last 1 -2 year took vitamins etc as prescribed by doctor during hair loss treatment, hair transplant. All stopped since March 2016. Can that may be an issue ??

Contd .. (was unable to write more through XXXXXXX app)

8) Not visited GP/Cardiologist yet. But shown yesterday report to MBBS doctor where I had gone for checkup. He advised exercise/ no junk food. Didn't get much information, so contacting here.

SUMMARY: I am a normal guy with average height and weight. I do not drink alcohol/ smoking. Not much habitual of eating junk food/ or visiting restaurant.
We do not prepare oily-food/sweets at home. Do not like sweet much.

Only thing in past related to supplement, are 1-2 years regular vitamins medicine like (F-Cal d3, F-Trichogrow, F-Q10S, F-Trichoflax, F-Trichoprotein, F-TrichoFerrum etc) that too with consultation with my HAIR LOSS treatment doctor. It was stopped this March 2016.

Brief Answer:
follow as already advised along with the following advises..

Detailed Answer:
Dear XXXXXXX Good to hear from you,
Welcome Back,

I was expecting you to reply a lot earlier so that any delay can be avoided at the maximum, including those health concerns which I've already mentioned earlier,

Now, your answer contains one more key to your unexplained metabolic parameters, please let me know at the first place, what made you to use 'so many multivitamins and iron and even Co-Q supplements?' - was it only the hair-loss or some thing else ?

It's not normal to use so many supplementation in such a short time-span of 1-2 years..

My dear, even though you've mentioned that you're normal guy with avg. height and weight- let know the tentative parameters at least please so that i can calculate an actual BMI for you..

Now coming to your answers :

1&2 yes you must check for it at the earliest, however those reports you've uploaded, seems absolutely fine, arises more suspicion for other etiologies as well as further in detailed examination and further evaluation (please keep in mind that there can be machine calibration error too, but those are also really negligible now a days with the state of the art technology available)..

3 - Good

4. No it has to be increased as I've already explained, yes please do so..

5. No, egg (cholesterol) is a highly debatable topic so i don't want to go in there but please note that 2 eggs per day is absolutely fine at your age, regardless of those facts and believes..

6. Good, so it has nothing related to this..

7. this is the major concern now, so please get yourself checked with a proper Dermatologist(skin Specialist) and not just some GP/Dietitian or anybody else for this..

8. Not that won't suffice, had that been, you'd have been recovered already..

9. I've seen your 11th June's report as attached herewith, but this link you've provided is not opening, anyway if both are same then you can ignore it..

First thing's first, you must get evaluated by a Cardiologist along with your health reports(both past and present), and get clinically examined to determine on the further course of actions (if those parameters are accurate, then i'd strongly recommend Lipid lowering therapy in form of statin group of drugs), meanwhile maintain all those I've already mentioned earlier..

You must get evaluated for Diabetes(i.e. FBS, PPBS, HbA1C) as well as ultra-sonography of whole abdomen with special focus on Liver, if it comes back normal, great relief..

Let me know dear, how you stay.. please keep me updated on your progress..

Take Care
Kind Regards

Hello,

Thanks for reply. Regarding (7) vitamins etc, the doctor was well qualified and I had also raised the concern on medicine in past, but he said they are OK.
I would contact him again.

I am not sure which 11-june report you have referred. Was that from my earlier queries with other doctors (In last few months.).

Brief Answer:
all of your visible reports are well evaluated..

Detailed Answer:
Dear XXXXXXX Welcome Back,

I've checked your Reports as per your descriptions,
✓ Image_1.png has been uploaded.
✓ Image_2.png has been uploaded.
✓ Image_3.png has been uploaded.
✓ Image_4.png has been uploaded. - and it seems your BMI is just about the higher baseline for being leveled as overweight, not to worry as it's not only controllable but also reversible, so just follow those previously mentioned measures..

My Dear, You need to understand that, due to being text based query, I won't be able evaluate if you just tell me "the doctor was well qualified", as it'll leave me grossly undetermined whether you've understood what i'm trying to explain to you in the first place, as well as the need for relevant specialist (cardiologist in this case), please try to understand..

Yes, I'm well aware of those reasons why those medications were prescribed in the first place that's why raised those questions for their relevance..

Your most welcome to go ahead with your Queries for your doctor's evaluation..

Let me know how you stay in the due course..

Take Care
Kind Regards

it's a Dermatologist dear(hair loss treatment doctor), and once again i'm informing you that he/she's not relevant, as in your case!!

Detailed Answer:
Hello Dear,
Welcome Back,

Yes, Please get a consultation with the cardiologist in the first place and not the dermatologist (simply as it's not relevant in your case) as i've explained to you numerous times..
