Suggest Treatment To Remove Cyst In Liver

Question: My mother i having 4 cysts on her liver of the sizes mentioned below

173*128 mm
73*69 mm
38*38 mm
49*35 mm

She was having pain near her liver and the doctor she consulted gave her antacid and gastric medicines to decrease her pain.after eating those medicine her pain went away for 4 months and again the same pain has started when she stands for long time.Please let us know if antacids are correct medicine for the pain and does this size of cyst need to be operated and removed?

Hi,

Welcome to HealthcareMagic .

The size of cyst which you have mentioned are quite large . They are stretching the liver covering and causing pain. With antacids it is unlikely that pain will reduce. They need to be operated.

The definative treatment will depend on the following things which I like to know from you
- is the cyst contain clear fluid?
-what are location of cyst ,means which lobe of liver
- does cyst contain septations .
-does cyst communicate with nearby structure .

If these details are not given in ultrasound report then CT scan of abdomen to be considered. If possible I advice you to send me picture of reports which have been done till now.

Ct scan is mainly required to diagnose it is a cancer cyst or not. To know thickness of cyst wall and also location to its surrounding structure.

Hi,

After reading the CT report I conclude that they are simple cysts and not a cancer cysts . Also it is an uncomplicated cysts wherein the fluid is clear and wall is not too thickened and also surrounding structures normal.

There are 2 options for treatment one is aspiration by passing needle from skin over liver and another is laparoscopy.

I personally prefer and advice you to go for laparoscopy after discussing with your treating general /gastro surgeon ( preferably gastro surgeon). Here the cyst wall is half removed and rest eroded so that again cyst won't form.

In case of aspiration chances of recurrence are high. No drugs are proved to resolve cyst. They are just to relieve symptoms like pain.

This report is around 4 months old.so within what time limit my mother should go for laparoscopy?is this operation complicated ?if you can brief me about this operation and approximate cost in market and suggest for any good hospital in XXXXXXX then will be really greatful.if this cyst are not operated is there any harm to my mother's life?

its a simple non-emergency operation

Hi,

There is no time limit as such. These are simple cyst and so its not an emergency to get these operated. You can plan the surgery at your convenience after consulting a gastro specialist.

These cyst are not life threatening, but as they will increase in size, the pain will increase and if the will compress upon the bile ducts, may lead to jaundice.

This surgery if done laparoscopically will be done under general anaesthesia, and is not a very complicated procedur. As told before, here the cyst wall is half removed and rest eroded so that again cyst won't form.The hospital stay may be about 2 days of hospital stay before surgery and 3-4 days after it.

The surgery in XXXXXXX can be done at various good hospitals like Manipal Hospital, Apollo Hospitals, XXXXXXX Asia Hospital, St. John's College Hospital, XXXXXXX Hospital, and all hospitals attached to a medical college.

The cost will vary from 10-15 thousands in a medical college hospital to 50-60 thousands in a private corporate hospital, depending upon the room you take.

As suggested by you,my mother showed to an gastro specialist in my hometown XXXXXXX The doctor asked my mother to do a number of blood test whose reports are attached above.please go through these reports and let me know if any deviations found from the normal range.

After seeing this report and the CT scan which i had shown you earlier,the doctor said that no surgery is required as of now and he asked my mother to eat the tablet ursocol300 for 2 months and after that come again for checkup.please let me know what this tablet is for and will this fix the root cause of the disease.

Hi,

I went through all the blood report which you had sent. In that everything is in normal range apart from fasting blood sugar and ALP. These both are more then the normal range .

Urosocol is mainly used to reduce cholesterol level and hence dissolve gall bladder stone and also reduces, if there is, increase in bilirubin levels. But in your reports bilirubin level is normal.

Effect of this drug on the cyst , I frankly have no idea how it will help. As far as i know and i have read there is no role of urosocol on reduction of cyst size. Regarding this you have to ask the doctor who has prescribed this.

As you have mentioned that the fasting blood sugar and ALP. These both are more then the normal range.does this both being out of normal range will have effect on the body and the cyst?what precaution should be taken to bring these two things in normal?

As there are no good hospitals in my hometown,i am planning to do the laproscopy of my mother once she comes to XXXXXXX in may 2015.but still 10 months more in that.So can she wait for 10 months and then operate the cysts?will the cyst has chance of enlarging in this 10 months time?kindly suggest.

Hi,

A raised fasting blood sugar level points towards diabetes, if she is not a known diabetic, then get a post prandial blood sugar levels or preferably a glucose tolerance test to confirm the diagnosis so that proper medication can be started for the same.

If she is a know diabetic then the drugs dose need to be adjusted for better glycemic control, in consultation with her physician.

The ALP could have been raised due the cyst compressing the bile ducts, but it is not raised that much to suggest any serious obstruction.

In 10 months the cysts will definitely increase in size, but how much they will grow, can't be predicted. That will need regular follow up by usg.

So keep monitoring her till she comes to you.

My mother does not have any diabetic background from before.This is the first time her sugar level is showing high.Is this level very high to be called as diabetic?
She is getting tiered very soon.eating less but gaining weight and not having good appetite these days.are this because of cyst or high sugar level?

Can you please describe in detail what is post prandial blood sugar levels or preferably a glucose tolerance test?what are the benefit of doing this test?

Hi,

Glucose tolerance test or a combination of fasting and post prandial (post meal) sugar levels are done to diagnose diabetes in an individual. Glucose tolerance test is better of the two.

Just a single raised fasting level is not enough to label as diabetic, so the above tests are done to confirm.

The other symptoms you mentioned are not of diabetes but are usually seen in hypothroidism. So it will be better to get her thyroid profile done too.
