Suggest Treatment Options For Cirrhosis Of Liver

default
Posted on Tue, 7 Oct 2014
Question: I have recently been diagnosed at the XXXXXXX Clinic in Ohio with Cirrhosis of the liver due to long term hepatitis C. I am about to start on a regimen of Sovaldi (Sofosbuvir) and Olyssio (Simeprevir) which has been shown to have a cure rate of 92%..
I am in the 4th stage of Hepatitis C but still with enough functioning liver to live a normal life. I am told that if the medication cures the Hep C this will stop the progression of the cirrhosis but it is possible for the Cirrhosis to progress on its own.
I don’t have many of the symptoms you would likely see if your liver were losing any ability to function and you would then be a candidate for liver transplant. I am referring to ascites, esophageal varices, spider veins, Jaundice, red hands, edema, etc. I don’t have any of that. I do have weight loss of 25 lbs in the past 6 months (current weight 194) muscle atrophy and enlarged spleen. My viral load is 6.8, I still have a great appetite and even though I’ve a lot thinner I am not more tired than usual and I feel great! I have asked all the questions below to the medical personnel I am working with in XXXXXXX but just want to get another opinion.

My questions:

1.     Would you concur with the opinion that it is possible for the progression of the cirrhosis to stop and never progress over many years.

2.     Is it possible to live for 10 or 15 years lets say, on the 10 or 20% liver still intact?

3.     Have you ever seen or heard of a case of cirrhosis of the liver reversing itself and regenerating at least a portion of the liver?

4.     When the Hep C is cured will It be possible to regain the lost weight, build up my muscle again? Will my spleen go back to its normal size?

5.     Based on the limited information I have given you what kind of prognosis would you give me?

Thanks much!

Brief Answer:
Cirrhosis is a irreversible stage.

Detailed Answer:
Hello XXXX

I have gone through your reports in detail.
Sorry to know about illness.

Progression of ongoing fibrosis in liver(cirrhosis) can be limited by taking proper treatment for hepatitis C.You are rightly advised for antiviral therapy.Progression of cirrhosis may be stopped if your condition is treated completely.

Liver has huge capacity and it can function well with remaining healthy cells, but exact longevity is difficult to say.It depend upon many factors.

Cirrhosis is a irreversible stage but we can limit its progression if underlying cause is treated.

You will definitely have weight gain after treatment .Enlarged spleen will regress in size after treatment but it willn't reverse to its normal size.

From the information provided,we can expect good prognosis.

Get well soon.
Hope I have answered your question.
Further queries are most welcome.
