Suggest Treatment For White Matter Lesions

Question: Hello, I just had an MRI done today to scan for a possible but unlikely mini-stroke; I had been having what I’m certain are panic/anxiety attacks which began with a very high stress episode about a month ago, with a deadline at work along with other obligations and a pending, major life/career change. These included severe bouts of lightheadedness, chest pains, and in particular shooting/sharp pain in my left arm that happens abruptly then goes away (usually the upper tricep and a couple times in the wrist). I just had a treadmill stress test and echocardiogram today as well, which were fine. But with the MRI, I got a call back quickly and was informed I had 15 mini-lesions, which the Dr thought might mean a microvascular issue or demyelination...what insights can you give me and is it possible the lesions might be benign? What am I facing? I asked this question earlier on here, but without the history leading up why the MRI was ordered, and that doctor opined it might be a benign white matter disease, but he pointed out he didn’t have a history to further inform that opinion, so I’m hoping the backstory helps. (I also have a history of high blood pressure.) Thanks.

Brief Answer:
Go for further tests

Detailed Answer:
Hi
I am Dr Mittal
I have read your query

From the query, I surmise that you basically need to know if the MRI changes are significant and should you do anything about it.

I think you should take this seriously
Ideally I would like to see the images before commenting.
But still-
The white matter lesions, especially in hypertensive patients is a cause of stroke.
Major strokes can result in these patients.
Therefore a complete evaluation of the stroke is advisable.

Also, the changes may be because of hypertension itself.
So the blood pressure needs to be controlled well.

I would suggest further tests like MR angiogram and CV doppler.

I have tried to answer your question as accurately as possible.

Best of luck
