Question: hi, few days ago i went surfing and one wave smashed me, flipped my board and pushed the fins in my backside. i felt strong pain right after but i ignored it and continued to surf. when i got home i realized that i was bleeding from my vagina. i took a look in the mirror and found two deep holes on each side of my labia minora. it's swollen and bleeding. i cleaned it with warm water and i waited. its the third day now and the cuts are looking a bit better. i'm wondering how long will it take to heal and should i b concerned and visit a doctor? how can i make it heal faster? thank you

Hello and welcome,

I am Dr Madhuri and will be answering your queries.

I have checked the pictures and the cuts are fairly deep. You must have had a lot of pain.

As of now, if the bleeding has stopped, they will heal by themselves. However I am concerned as one of the cut [ the upper one] looks fairly close to the urethra. Are you able to pass urine properly? or are there any problems. The urethra is not very clearly seen in the picture. So if you are able to upload an image where the urethra is seen properly [ preferably keep a finger on the urethra to point it out] or a closer image then it will help.

If the cuts are very close to the urethra or involving the urethra then you will need to see a doctor and get evaluated as it may cause stricture formation [ narrowing of urethra after healing] and can be prevented by timely intervention.

The cuts actually needed a suture to help them heal quicker. But since it is already 3 days, we do not prefer to suture them so late. If the cuts are away from the urethra and there is no problem with urination, then you just need to wait. You may use placental extract ointment if available to help in faster healing. There is no sign of infection so just keep the area approximated and do not touch or move it repeatedly to help healing. Change the undergarments twice daily to help prevent infection and do not have sex until there is complete healing. They will take about 7-10 days to heal provided there is no infection. Cleaning daily once with antiseptic wipes also helps.

Use a pain killer if there is a lot of pain like acetaminophen with serratiopeptidase to decrease swelling.

Please do not worry as most likely the urethra is not involved as you did not mention any urinary complaints. Just let me know the details that I needed to help you further.

hi, thank u 4 responding so quickly. i am experiencing something strange. i pee a lot more times and each time feels like i am not able to empty my bladder. i pee small amount each time and need to go to the toilet a lot. and i also feels dull pain on my right side lower back like when im having my period except i'm not. i also feel slight pain above my vagina under belly bottom. is it really serious than i thought? if i need to go to the hospital what should i ask the doctor to do? as i'm living in China right now and the hospitals here can b really behind with the sports injuries. i dont want to b misdiagnosed by them. i need to tell them exactly what i need from them.
please help i'm terrified.
p.s these r the pix of today, looking better but im worried about the urethra thing that u mentioned
and i also feel extremely tired and exhausted these two days

I have seen the pictures. Please do not worry so much. Such injuries are not uncommon and every gynecologist deals with them.
The urethra is the place from where the urine comes out. From the pictures it seems that the upper injury is close to this place. But I am not able to see the urethra properly. So you will need to go to a doctor [ preferably a surgeon or a gynecologist ] and then get it confirmed. A catheter is passed from the urethra to confirm that it is intact. So just ask them to check for the urethra if it is torn or not. Also if there is a tear it is confirmed by identifying the external urethral orifice and any surgeon or gynecologist will be able to do it.

You can also try to check it by trying to pee and watching in the mirror. If the urine is coming out and this place is away from the tear then the urethra is not involved. If the urine is coming out from the torn part then a urethra tear is there. In any case I suggest that you see a doctor as you are having urinary complaints. Drink plenty of water and an antibiotic like nitrofurantoin will help cure any urine infection.

Please do not worry. All tears heal and yours have already started to heal. So just get an evaluation. The exhaustion and tiredness may be due to the injuries or a urine infection. Take tablet acetaminophen [ commonly sold as tylenol or paracetamol] with milk to help cure pain and provide relief.
