Brief Answer:
Recurrent stricture should be treated by an experienced surgeon.

Detailed Answer:
Hello XXXX and welcome to HCM.
As an Urologist, i can fully understand your concern.

Yes the X-ray pictures are very important to comment about treatment.
Waiting for 20 years is a very long time, after a failed surgery.
Buccal mucosa graft ( from inside mouth), is one of the commonly used grafts.
Recurrent stricture is also difficult to treat, hence always get it done by an experienced surgeon.
If successful, semen should come out after surgery, following sex.

Wish you well.

Brief Answer:
It's a severe stricture.

Detailed Answer:
Hi again.
Just went through your urethrogram X-ray.
It shows a very severe stricture, in the urinary tube below the bladder.
Stricture means an area of narrowing.
It will need an expert to, operate this successfully, after which, the catheter in the bladder can be removed, and you should be able to pass urine freely.
As recurrent stricture is well known, another surgery is then needed.
Wish you well.

Brief Answer:
Sex should be possible with semen emission, after surgery.

Detailed Answer:
Hi again.
After a successful surgery in one stage, a person can usually have normal intercourse with semen ejaculation.
At present, you can very well have sex, but semen may not be coming out, due to block in the urinary tube or stricture.
In an experienced surgeons' hands, this is done as a single stage.
What the hole in the end of penis is, couldn't understand.
Clarify with your surgeon, his prior patient's experience.
Find out if the surgeon is experienced, and if similar patients' have been operated there. Some centers' do it in two stages.
Hope your doubts are cleared. Wish you well.

Brief Answer:
Semen doesn't get stored up with the block.

Detailed Answer:
Hi again.
After a successful surgery, person will be able to have normal intercourse, with semen ejaculation, at the end of it.
No pain is felt.
The semen which formed so far, gets absorbed and doesn't keep getting stored there.
Fresh semen will form and come out after sex.
Hope your doubts are cleared.
You're welcome to ask any doubt you have.
Wish you well.
