Suggest Treatment For Uncontrolled Hypertension

Question: I have very labile hypertension-I assume that it is isolated systolic hypertension. I am on diltiazem 120 sustained extended and 75 mgm of avapro. Also 60 mgm short acting diltiazem before bed. I was in florida for 6 months and came home last week. For the past 6 nights my blood pressure wakes me up with systolic readings about 190. Diastolic is always normal -70-80. The weather in florida was very warm in the high 80's and in NY State it has been very cold. In Florida, my blood pressure got so low that they took me off the avapro and hydrochloride . I maintained a blood pressure there ranging from 100-120 and very occasionally 130 systolic with just the long acting diltiazem. I am awakened every hour or two . I instantly know that my blood pressure is up and have symptoms of pain in the back of my head , feeling nauseated and sometimes shaking and dizzy. I am voiding large amounts of urine at night sometimes reaching 1500 cc.
much more than what I drink during the day. My question is what kind of treatment could I get/ At this point I am so sleep depleted because I only sleep in short amounts. Thank you in advance

Hello!

Welcome and thank you for asking on HCM!

I passed carefully through your question and would explain that your blood pressure values are really high, which indicates uncontrolled hypertension and I don't think that these changes are only related to the weather change.

The massive urinating during the night could indicate a possible kidney disease, which is a known rosk factor for uncontrolled hypertension.

For this reason, I would recommend consulting with your doctor for a careful physical examination and some tests to investigate for the possible causes underlying your uncontrolled hypertension :

- a resting ECG
- a chest X ray study
- an abdominal ultrasound to examine your urinary tract and kidneys
- complete blood count
- kidney and liver function tests
- fasting glucose
- blood electrolytes
- thyroid hormone levels
- an urine analysis.

You should discuss with your doctor on the above issues.

Hope to have been helpful!

Kind regards,

Dr. Iliri
